摘  要:目的了解山东省某医院体外循环心脏直视手术后呼吸机相关性肺炎的致病菌分布情况及药物敏感性。方法2017年3月回顾性分析该院2007年1月至2017年1月接受CPB心脏直视手术的392例病人体外循环心脏直视手术后呼吸机相关性肺炎患者下呼吸道分泌物所分离的致病菌,并进行药物敏感实验。结果分离出致病菌89株,其中致病菌以革兰阴性菌为主68株(占76.40%),其次为真菌15株(16.85%),革兰阳性(G+)菌6株(6.74%)。部分菌株对抗生素存在耐药性。结论体外循环心脏直视手术后呼吸机相关性肺炎患者的致病菌以革兰阴性菌为主,临床医生在治疗过程中应合理选用抗生素以提高手术疗效。Objective To understand the pathogenic microbial strains and drug sensitivity of ventilator-associated pneumonia after cardiac surgery with cardiopulmonary bypass in a hospital of Shandong province. Methods In March 2017, 392 cases who underwent cardiac surgery with cardiopulmonary bypass were collected in the hospital,January 2007-Janu- ary 2017. The distribution and drug sensitivity pattern of the pathogenic microbial strains isolated from lower respiratory tract aspirations were analyzed retrospectively. Results A total of 89 pathogenic microbial strains were isolated. Gramnegative bacteria were the most, 68strains (accounting for 76.40%), followed by fungi, 15 strains (accounting for 6.85 % ) and Gram-positive bacteria,6strains (accounting for 6.74%). Some bacteria were resistant to antibiotics. Conclusion Gram-negative bacteria are the most in pathogenic microbial strains in the cases who underwent ventilator-associated pneumonia after cardiac surgery with cardiopulmonary bypass,and effective antibiotics should be chosen by the doctors in the clinical practice to improve the operation quality and clinical effect.
