冠心病患者颈动脉弹性的超声评估及其与斑块稳定性、血小板功能的相关性

摘　　要：目的:研究冠心病患者颈动脉弹性的超声评估及其与斑块稳定性、血小板功能的相关性。方法:选择稳定型心绞痛患者、不稳定型心绞痛患者以及健康志愿者,分别作为SAP组、UAP组、对照组。进行颈动脉超声检查并测定单点脉搏波传导速度(PWVβ);采集血清及外周血标本,测定斑块稳定性相关指标及血小板功能相关指标。结果:SAP组和UAP组患者的颈动脉PWVβ水平、血清中ICTP、MMP1、MMP3、MMP7、CatK、CD62p、CD63含量以及外周血中PAR1、LOX1的表达量均显著高于对照组,UAP组患者的颈动脉PWVβ水平、血清中ICTP、MMP1、MMP3、MMP7、CatK、CD62p、CD63含量以及外周血中PAR1、LOX1的表达量均显著高于SAP组;PWVβ高水平冠心病患者外周血中PAR1、LOX1的表达量以及血清中CD62p、CD63、ICTP、MMP1、MMP3、MMP7、CatK、CD62p、CD63的含量均显著高于PWVβ低水平冠心病患者。结论:冠心病心绞痛患者颈动脉弹性的超声参数PWVβ对斑块稳定性及血小板功能具有评估价值。Objective：To study the ultrasound assessment of carotid artery elasticity in patients with coronary heart disease and its correlation with plaque stability and platelet function.Methods：Patients with stable angina pectoris,patients with unstable angina pectoris and healthy volunteers were selected and included in SAP group,UAP group and control group respectively.Carotid ultrasonography was conducted to determine single point pulse wave velocity（PWVβ）;serum and peripheral blood samples were collected to determine plaque stability-related indexes and platelet function-related indexes.Results：Carotid artery PWVβlevel,serum ICTP,MMP1,MMP3,MMP7,CatK,CD62 p and CD63 contents as well as peripheral blood PAR1 and LOX1expression of SAP group and UAP group were significantly higher than those of control group,and carotid artery PWVβlevel,serum ICTP,MMP1,MMP3,MMP7,CatK,CD62 p and CD63 contents as well as peripheral blood PAR1 and LOX1expression of UAP group were significantly higher than those of SAP group;peripheral blood PAR1 and LOX1expression as well as serum CD62 p,CD63,ICTP,MMP1,MMP3,MMP7,CatK,CD62 p and CD63 contents of coronary heart disease patients with high PWVβlevels were significantly higher than those of coronary heart disease patients with low PWVβlevels.Conclusions：The carotid artery elasticity ultrasound parameter PWVβhas assessment value for the plaque stability and platelet function in patients with coronary heart disease angina pectoris.
