远端蒂掌背皮瓣修复手指深度烧伤创面

摘　　要：目的探讨远端蒂掌背皮瓣一期修复手指肌腱、骨外露创面的临床效果。方法自2010年1月至2015年5月,应用掌背动脉逆行岛状皮瓣及掌背皮神经营养血管皮瓣修复近中节手指烧伤创面共18例;2种方法的远端蒂掌背皮瓣切取面积为2.5 cm×1.5 cm^5.5 cm×2.5 cm,供区直接缝合或植皮覆盖。结果本组共18例患者,术后1例皮瓣远端皮肤有淤血、水疱形成,经换药后2周愈合;其余患者术后随访3周至12个月,皮瓣血运良好,手指外形、功能及感觉均满意。结论采用远端蒂掌背皮瓣修复手指近中节深度烧伤创面,血供可靠,可以一期修复,且供区损伤较小,适合基层医院推广应用。Objective To explore the application of distally-based dorsal metacarpal flaps to repair tissue defects of the fingers with exposed tendon or bone.Methods From January 2010 to May 2015,distally-based dorsal metacarpal flaps,including the reversed dorsal metacarpal artery flap and neurocutaneous flap,were successfully used in deep burn wound repair of the proximal finger for 18 patients with burn wounds.The incision area of the distally-based dorsal metacarpal flap in both methods ranged from 2.5 cm × 1.5 cm to 5.5 cm × 2.5 cm.The donor sites were sutured directly or covered with free skin grafts.Results follow-up of 3 weeks to 12 months,only one complication occurred with stagnated blood and blister in the distal flap,which healed after changing dressings for two weeks.The other flaps had good blood circulation.The patients felt satisfied with postoperative appearance,function and sensation.Conclusion Distally-based dorsal metacarpal flaps with reliable blood supply can be used to repair deep burn wounds of the proximal and middle phalanx finger.The flap is advantageous in that the injury to the donor site is minor and the flap is easily manipulated.This technique is suitable to popularize in clinic.
