摘 要:目的探讨抗生素骨水泥治疗跟腱断裂合并感染的早期疗效。方法回顾性分析自2012年1月至2016年1月沈阳军区总医院收治的40例跟腱断裂合并感染患者的临床资料,将2012年1月至2013年12月收治的患者作为A组,2014年1月至2016年1月收治的患者作为B组,每组各20例患者。A组采用传统一期伤口清创,无菌敷料包扎伤口后行抗感染治疗,B组伤口清创后,跟腱缺损处加入抗生素骨水泥。比较两组平均住院时间、感染控制时间、创面痊愈时间及创面软组织生长情况。结果 B组患者平均住院时间、感染控制时间、创面痊愈时间均较A组明显缩短,两组间比较,差异均有统计学意义(P<0.01);B组创面软组织生长良好19例(95.0%)高于A组的12例(60.0%),两组间比较,差异有统计学意义(P<0.05)。结论抗生素骨水泥治疗跟腱断裂合并感染的早期疗效确切,具有缩短感染控制时间、减少住院天数、促进创面愈合及改善创面软组织生长等特点。Objective To investigate the effect of early antibiotic bone cement for the treatment of Achilles tendon rupture with infection.Methods A retrospective study was performed on 40 cases of Achilles tendon rupture with infection who were admitted from January 2012 to January 2016.Patients who were admitted from January 2012 to December 2013 were divided into Group A,the others who were admitted from January 2014 to January 2016 were in Group B,with 20 cases in each group.Patients in Group A received conventional one stage debridement,aseptic dressing wounds after anti infection treatment,the others in Group B received debridement,antibiotic bone cement were added in Achilles tendon defect.The average hospitalization time,infection control time,healing time and evaluation of growth the wound of soft tissue were compared between the two groups.Results Compared with Group A,the average hospitalization time,infection control time,and healing time in Group B were significantly shorter(P〈0.01);the soft tissue wound good growth rate in Group B(95.0%) was higher than that in Group A(60.0%),all the differences had statistical significance(P〈0.05).Conclusion The curative effect of early antibiotic bone cement for the treatment of Achilles tendon rupture with infection is effective,which can shorten the time of infection control,reduce hospitalization time,promote wound healing,and improve the characteristics of wound and soft tissue growth.
