摘 要:目的探讨颅脑损伤患者Homer1a蛋白表达及其与神经元损伤、凋亡的关系。方法选择武汉市第一医院神经外科自2012年5月至2016年3月收治的颅脑损伤患者42例(颅脑损伤组)和同期本院门诊的健康体检者50例(正常对照组),采用酶联免疫吸附测定法(ELISA)检测受试者外周静脉血Homer1a蛋白和神经元损伤指标(神经元特异性雌烯醇化酶(NSE)、肝型脂肪酸结合蛋白(FABP)、胰岛素样生长因子(IGF-1)、S100B蛋白1的含量,采用放射免疫法检测神经元凋亡指标[可溶性凋亡相关因子(sFas)、可溶性凋亡相关因子配体(sFasL)、细胞淋巴瘤-2基因(Bcl-2)]的含量。结果颅脑损伤组患者血清中Homer1a蛋白的含量[(113.27±12.19)pg/mL]明显高于正常对照组[(53.93±4.06)pg/mL],差异有统计学意义(P〈0.05)。颅脑损伤组患者血清Homer1a蛋白含量中位数为115.302pg/mL,以此为界将颅脑损伤组患者分为高Homer1a蛋白组、低Homer1a蛋白组。统计显示低Homer1a蛋白组、高Homer1a蛋白组、正常对照组血清NSE、FABP、S100B、sFas、sFasL含量依次降低,IGF-1、Bcl-2含量依次增高,差异均有统计学意义(P〈0.05)。结论颅脑损伤患者血清Homer1a蛋白表达增加,其含量高低与神经元损伤、凋亡指标有关。Objective To study the Homerla protein expression and its relationship with neurological deficit and neuronal apoptosis in eraniocerebral trauma patients. Methods Forty-two craniocerebral trauma patients, admitted to our hospital from May 2012 to March 2016, were selected as craniocerebral trauma group; 50 healthy subjects accepted physical examination at the same period in our hospital were selected as normal control group (n=50). Immediately after admission, serum contents of Homerla protein and nerve fimction damage indices (neurospecific estrogenase [NSE]), fatty acid binding protein [FABP], insulin-like growth factor [IGF-1], and SI00B protein) were measured by enzyme linked immunosorbent assay (ELISA). Serum apoptotic indices (soluble apoptotic factor [(sFas)], sFas ligand [sFasL], and cell lymphoma-2 [Bcl-2]) were detected by radioimmunoassay. Results Immediately after admission, serum content of Homerla protein content in craniocerebral trauma group ( [ 113.27±12.19] pg/mL) was significantly higher than that in normal control group ([53.93±4.06] pg/mL, P〈0.05); the median serum Homerla protein level was 115.302 pg/mL, and according to this level, the patients from the craniocerebral trauma group were further divided into high Homerla group and low Homerla group. Serum NSE, FABP, S100B, sFas and sFasL levels in the high Homerla group, lowHomerla group and normal control group were decreased in sequence, and IGF-1 and Bcl-2 levels increased in sequence, with significant differences (P〈0.05). Conclusion Expression of Homerla protein is increased in patients with traumatic brain injury, and its content is directly related to nerve injury and neuron apoptosis.
