盆底肌肉锻炼联合电刺激对广泛全子宫切除术后患者的影响

摘　　要：目的:探讨盆底肌肉锻炼联合电刺激对广泛全子宫切除术后患者的影响。方法:将120例广泛全子宫切除术患者根据入院时间分为研究组和对照组各60例,对照组实施常规护理,研究组在对照组基础上实施盆底肌肉锻炼联合电刺激,比较两组术后并发症发生率、导尿管留置时间、残余尿量及盆底肌力分级状况。结果:研究组尿路感染、尿潴留、排尿困难、重置导尿管1次的发生率低于对照组(P<0.05),导尿管留置时间及首次拔管残余尿量少于对照组(P<0.001),术后21 d盆底肌肉分级显著高于对照组(P<0.05)。结论:对广泛全子宫切除术患者进行围术期盆底肌肉锻炼联合电刺激,能促进其自主排尿功能的恢复,缩短留置导尿管时间,降低尿潴留及尿路感染发生率,改善盆底肌力。Objective：To investigate the effects of pelvic floor exercises combined with electrical stimulation on patients after extensive hysterectomy. Methods：A total of 120 cases of extensive hysterectomy were divided into the study group and the control group according to the time of admission,60 cases in each group. The control group received routine nursing care, and the study group implemented pelvic floor muscle exercises combined with electrical stimulation on the basis of the control group. The incidence of postoperative complications, indwelling time of urethral catheter,residual urine volume and pelvic floor muscle strength classification were compared between the two groups. Results：The study group＇s incidence rates of urinary tract infection, urinary retention, dysuria, reset the catheter once were signifi- cantly lower than tt^ose in the control group （ P 〈 0.05 ）. Catheter indwelling time and first extubation residual urine volume were signifi- cantly less than those of control group （ P 〈 0.001 ）. The pelvic floor muscle strength classification on 21 days after the operation was sig- nificantly higher than that of the control group （P 〈 0.05 ）. Conclusion ： The pelvic floor muscle training combined with electrical stimula- tion for extensive hysterectomy patients during the perioperative period can promote the recovery of automatic mieturition function, shorten the time of indwelling catheter, reduce the incidence of urinary retention and urinary tract infection, and improve the pelvic floor muscle strength.
