内镜逆行胰胆管造影联合腹腔镜胆囊切除术治疗胆囊结石合并胆总管结石的临床分析

摘　　要：目的观察内镜逆行胰胆管造影(ERCP)联合腹腔镜胆囊切除术(LC)治疗胆囊结石合并胆总管结石的临床效果。方法选取2013年12月~2016年12月在我院治疗的胆囊结石合并胆总管结石患者100例作为观察对象,采用抽签的方式将患者简单随机分为观察组和对照组,每组各50例。对照组患者LC+腹腔镜胆总管探查术(LCBDE)治疗,观察组患者采用ERCP联合LC进行治疗。将两组患者的手术时间、住院时间、住院费用、结石清除率、手术成功情况以及并发症发生的概率进行比较分析。结果观察组患者的手术时间、住院时间、住院费用均低于对照组,P<0.05;治疗后,观察组患者的结石清除率为94.00%,一次性手术成功率为76.00%,均高于对照组(80.00%、54.00%),P<0.05;且观察组患者并发症发生的概率为8.00%,较对照组(24.00%)低,P<0.05。结论ERCP联合LC治疗胆囊结石合并胆总管结石患者的效果更好,结石清除率高、住院时间短,值得临床推广。Objective To observe the clinical effect of ERCP combined with LC in the treatment of gallstones with choledocholithiasis.Methods 100 patients with gallstones combined with choledocholithiasis（December 2013 to December 2016） were selected as our subjects.All patients were randomized to observaction group and the control group,including 50 patients in each group.LC ＋LCBDE in patients was given with laparoscopic cholecystectomy of control group ERCP＋LC） was given in patients of observation group for treatment.The operative time,hospitalization time,hospitalization cost,stone clearance rate,operation success and the probability of occurrence of complication were compared between the two groups.Results The operation time,hospitalization time and hospitalization cost of the patients with choledocholithiasis combined with choledocholithiasis were lower than those in the control group（P〈0.05）.After treatment,the removal rate of stones was 94.00%,the success rate was 76.00% of the observation,and 80.00%,54.00% of the control group,P〈0.05.The probability of occurrence of complications in the observation group was 8.00%,which was lower than that in the control group（24.00%）,P〈0.05.Conclusion ERCP combined with LC in the treatment of gallstones with choledocholithiasis is more effective,high stone clearance rate,shorter hospital stay,and is widely popularized.
