儿童重度社区获得性肺炎相关危险因素的病例对照研究

摘　　要：目的探讨儿童重度社区获得性肺炎(CAP)的危险因素。方法选取2015年1月—2016年10月在笔者所在医院接受住院治疗的重度CAP患者320例为病例组,对照组为同期在院接受治疗的非重度CAP患者320例,对可能引起儿童重度CAP的临床相关因素进行单因素检验及多因素Logistic回归分析,以调查研究引起儿童重度CAP的危险因素。结果单因素分析显示年龄、居住地、先天性心脏病、既往反复感染、手术史、免疫缺陷、室内环境、按时接种麻疹疫苗差异有统计学意义(P<0.05),多因素Logistic回归分析结果显示年龄、手术史、免疫缺陷、先天性心脏病、室内环境等与儿童重度CAP的发病有关(P<0.05)。结论既往反复感染、手术史、免疫缺陷、先天性心脏病、室内环境污染为儿童重度CAP的危险因素,年龄、按时接种麻疹疫苗为保护因素。Objective To explore the risk factors of severe community acquired pneumonia （CAP） in children. Methods From January 2015 to October 2016 the admitted in author＇s hospital 320 cases of severe CAP were enrolled as case group,and at the same period 320 cases of children with non severe CAP as control one;the clinical related factors likely to cause children with severe CAP were taken univariate logistic test and multivariate regression analysis so as to investigate risk factors for severe CAP in children. Results Univariate logistic analysis showed that age, place of residence, congenital heart disease, previous repeated infection, surgical history,immunodeficiency,indoor environment,measles vaccation on time were statistically significant （P〈0.05）, multivariate logistic regression analysis showed age,surgical history,immunodeficiency,congenital heart disease, indoor environment and so on were related with onset of severe CAP （P〈0.05）. Conclusion Repeated infection, surgical history,immunodeficiency,congenital heart disease,indoor environmental pollution are risk factors for children suffering from severe CAP,while age,timely vaccinating measles vaccine could be as a protective factors.
