经输尿管镜置管治疗顽固性血精及精囊结石的效果观察

摘　　要：目的探讨经输尿管镜置管治疗顽固性血精及精囊结石的临床应用价值。方法选择2011年1月—2015年12月笔者所在医院收治的48例顽固性血精患者,随机分为观察组和对照组,各24例。观察组为微创方式治疗,采用经输尿管镜置管的方法,对照组采用传统经直肠超声引导下精囊穿刺置管,对比两组的疗效。结果观察组中21例行双侧精囊清洗,3例无法进入另一侧精囊而仅行单侧精囊清洗,其中18例精囊炎,6例合并精囊结石,观察组与对照组在平均手术时间、术中出血量、治愈率、复发率、术后精子密度和精子存活率指标、并发症发生率指标上均优于对照组,差异有统计学意义(P<0.05)。结论经输尿管镜治疗顽固性血精治愈率明显提高、并发症发生率显著减低,可降低血精复发率,疗效较好。Objective To explore the therapeutic method and clinically applied value to cure obstinate hemospermia and seminal vesicular stone with putting urethral catheter perurethra via ureteroscope. Methods The 48 patients with obstinate hemospermia which admitted in author＇s hospital from Jan.,2011 to Dec. , 2015 , were selected and divided them into observation group （n=24） and control group （n=24） at random. The observation group taken the minimally invasive surgery which was putting urethral catheter per urethra via ureteroscope. The control group taken the traditional surgery that was transperineal seminal vesicle puncture under TRUS guidance. The therapeutic effects of the two groups were compared. Results In the observation group,21 cases received the bilateral seminal vesicle cleaning,3 received the unilateral seminal vesicle cleaning among whom,18 were spermatocystitis and 6 were seminal vesicular stone. The results showed that the observation group was superior to the control group in average operating time,blood lose during operation,cure rate,receurrence rate,postoperative sperm density,sperm survival rate index and complication incidence rate,there was a significant difference （P〈 0.05）. Conclusion Curing obstinate hemospermia by ureteroscope could significantly improve the cure rate,reduce complication incidence,decrease hemospermia recurrence and has good efficacy.
