吗替麦考酚酯治疗坏疽性脓皮病1例及文献复习

摘　　要：患者男,60岁。右小腿疼痛性溃疡10年,双侧股骨头坏死8年。皮损组织病理示表皮增生,真皮浅层水肿和小血管增生,胶原纤维增生和纤维化,真皮内可见中性粒细胞、淋巴细胞、组织细胞等炎细胞浸润,可见大量含铁血黄素沉积。诊断:(1)坏疽性脓皮病;(2)股骨头坏死。采用吗替麦考酚酯(1.0g,2次/d)和对症治疗,获得一定疗效。同时对吗替麦考酚酯治疗坏疽性脓皮病的相关文献进行复习回顾。A 60-year-old man presented with a 10-year history of painful ulcers on the right lower leg anda 8-year history of bilateralfemoral head necrosis. Histological examination showed epidermal thickness,upper dermal edema,an increase in small blood vessels,collagen proliferation and prominent fibrosis. There was a mixed infiltrateofneutrophils,lymphocytes and histocytes in the dermis,accompanied by abundantdeposition of hemosiderin. A diagnosis of pyoderma gangrenosum associated with femoral head necrosis was made. Partial response was achieved with a combination of mycophenolate mofetil（ 500 mg twice daily） and symptomatic treatment. A review of the litera-ture concerning mycophenolate mofetil treatment for pyoderma gangrenosum is also presented.
