摘  要:目的分析北京市东城区2013~2015年手足口病(hand-foot-mouth disease,HFMD)病原学实验室检测结果及其流行病学特征。方法制备2013~2015年北京市东城区的HFMD疑似病例的咽拭子样本312份(包含监测样本225份及疫情样本87份),采用HFMD核酸检测试剂盒检测样本中的肠道病毒(enterovirus,EV)的型别,并采用细胞培养法对部分EV阳性样本进行病毒分离。结合实验室结果及病例的流行病学特征进行统计学分析。结果 225份HFMD监测样本中,EV阳性112份,其中EV71型阳性20份,柯萨奇病毒A16型(coxsackievirus 16,CA16)阳性37份,其他EV型阳性55份。2013年发生HFMD聚集性疫情3起,均由EV71型引起;2014年发生3起,分别由EV71、CA16和其他型EV引起;2015年发生22起,6起由CA16型引起,3起由EV71型引起,4起由CA6型引起,6起由其他型EV引起,3起由非EV引起。经病毒分离获得EV71阳性菌株10株,CA16型阳性菌株20株。结论北京市东城区2013~2015年HFMD仍以EV71和CA16为主要致病原,CA6阳性率呈上升趋势。应扩展病毒分型检测项目,更好地发现本地区HFMD病原的流行和变异趋势。Objective To analyze the laboratory testing results and epidemiological characteristics of hand-foot-mouth disease(HFMD) in Dongcheng District, Beijing City, China during 2013 ~ 2015. Methods A total of 312 throat swabs were collected from suspected patients with HFMD in Dongcheng Distruct during 2013 ~ 2015, including 225 monitoring samples and 87 epidemic samples, and identified for the type of enterovirus(EV)by HFMD DNA detection kit, based on which viruses were isolated from partial EV positive samples. The laboratory results were analyzed statistically in combination with the epidemiological characteristics of cases. Results Of the 225 monitoring samples, 112 were positive for EV, including 20 for EV71, 37 for CA16 and 55 for EV of other types. Three clustering epidemics occurred in 2013,all of which were caused by EV71. However, the three clustering epidemics in 2014 were caused by EV71, CA16 and EV of other types respectively. Of the 22 epidemics in 2015, 6 were caused by CA16, 3 by EV71, 4 by CA6, 6 by EV of other types, and 3 by viruses other than EV. Ten EV71 positive and 20 CA16 strains were isolated. Conclusion EV71 and CA16 were the major pathogen of HFMD in Dongcheng North District of Beijing in 2013, while the CA16 positive rate showed an increasing tendency. Virus typing should be performed to investigate the epidemic and variation tendencies of pathogens of HFMD.
