Atlas钛缆治疗粉碎性髌骨骨折的应用研究

摘　　要：目的:探讨Atlas钛缆治疗粉碎性髌骨骨折的临床疗效。方法:选取2015年5月-2016年10月住院的粉碎性髌骨骨折患者90例,采用数字随机法随机分3组治疗。其中Atlas钛缆内固定组(A组)30例,切开复位张力带钢丝内固定组(B组)30例,镍钛记忆合金聚髌器固定组(C组)30例。进行统计学分析比较这三组的内固定术所用时间、在手术中的出血量、膝关节在手术后第一次活动时间、住院总时间、住院总费用、手术并发症以及较远期膝关节功能情况。结果:A组30例中29例获得随访,B组30例中28例获得随访,C组30例中29例获得随访,随访时间为12~15个月,平均(13.02±0.36)个月。Atlas钛缆内固定组患者手术时间,住院时间最短,术中出血量最少(P<0.05);术后开始伸屈膝关节锻炼时间最早(P<0.05),但医疗费用较贵(P<0.05),约为1.32万元。本研究采用改良后的Bostman评估标准来比较三组手术后的远期疗效,结果显示A组(Atalas钛缆固定手术)的Bostman优良率为96.6%,结果显著比另外2组高,而且A组手术的并发症比另外2组少。结论:Atlas钛缆治疗髌骨粉碎性骨折,具有手术时间和住院时间短,出血量和并发症少、术后恢复快,恢复效果良好等优点,有着广泛的应用前景。To investigate the clinical effect of Atlas titanium cable in the treatment of comminuted patellar fracture.Methods：90 cases of comminuted patellar fracture in May 2015-2016 were randomly divided into treatment group and control group（n=3）.The Atlas titanium cable fixation group（A group） 30 cases,open reduction and tension band wire fixation group（B group） 30 cases,Ni Ti shape memory alloy patella concentrator fixation group（C group） 30 cases.Statistical analyses were performed to compare the three groups of internal fixation with time,intraoperative bleeding volume,postoperative knee joint in the first time,total hospitalization time and total hospitalization expenses,surgical complications and a long-term knee function.Results：in A group,29 cases were followed up,B group of 30 cases of the patients were followed up,C group of 30 cases of the patients were followed up for a period of 12 ~ 15 months,with an average of（13.02＋0.36） months（= 29）.Atlas titanium cable fixation group of patients operation time,hospitalization time shorter,intraoperative bleeding amount（P〈0.05）; postoperative knee extension and flexion exercise in the earliest time（P〈0.05）,but the expensive medical costs（P〈0.05）,about 13 thousand and 200 yuan.This study adopts Bostman evaluation criteria improved the long-term curative effect to compare the three groups after operation,the results show that the A group（Atalas titanium cable fixation） Bostman excellent rate was 96.6%,significantly higher than the other 2 groups,A group and surgical complications were less than the other 2.Conclusion：the treatment of comminuted patellar fracture with Atlas titanium cable has the advantages of shorter operation time,shorter hospital stay,less bleeding and fewer complications,faster postoperative recovery and better recovery effect.
