摘 要:目的分析湖北省2015年手足口病病原学特征,为手足口病科学防控提供依据。方法对手足口病临床诊断病例标本用实时荧光定量PCR法进行肠道病毒检测及EV 71、Cox A16特异性核酸检测。结果全年共检测粪便标本1 308份、咽拭子标本5 131份、疱疹液137份,粪便标本检测阳性率最高。2015年实验室确诊手足口病病例2 880例,其中EV 71、Cox A16和其他肠道病毒感染的构成比分别为21.0%、20.7%和58.3%。EV 71、Cox A16和其他肠道病毒均为男性病例多于女性.三者病例均以1~3岁组为主。4~6月为发病顶峰,Cox A16比EV 71和其他肠道病毒发病峰提前一个月。结论 2015年湖北省手足口病病原主要由其他肠道病毒引起,发病具有明显季节、人群特征。应做好重点人群、重点时段监测,同时进一步加强其他肠道病毒病原深入研究.Objective To investigate the etiologic and epidemic characteristics of hand-foot – mouth disease(HFMD) in Hubei Province in 2015,the outcome could support for evidence-based policy-making of the prevention and control of the disease. Methods Feces,throat swab or Herpes fluid specimens were collected from clinically diagnosed cases with HFMD. Real-time fluorescent PCR(RT-PCR) was used to detect enterovirus,enterovirus 71(EV 71) and coxsackievirus A16(Cox A16). Results A total of 1 308 stool samples,5 131 samples of throat swab,137 copies of herpes fluid were detected. Stool specimens had the highest positive rate. Laboratory confirmed cases of 2 880 cases in 2015,the constituent ratios of Cox A16,EV 71 and other enteric viral were 21. 0%,20. 7% and 58. 3%,respectively. Male' s morbidity was higher than that of female' s in Cox A16,EV 71 and other enteric viral. Most of the cases were children from 1 ~ 3 years old. 4 ~ 6 month was the peak of the disease. Cox A16 compared to EV 71 and other enteric viruses in the onset of peak a month in advance. Conclusion The major pathogen causing HFMD was other enterovirus in Hubei Province in 2015,the epidemic of the HFMD has obvious characteristics of season and population characteristics,so it is important to do well in the prevention and control of the focused period,and the targeted populations. At the same time,we further strengthen the further study of other intestinal viral pathogens.
