不同证型原发免疫性血小板减少症患者淋巴细胞亚群表达特征

摘　　要：目的比较不同中医证型原发免疫性血小板减少症(ITP)患者淋巴细胞亚群表达的差异性。方法收集186例ITP患者,中医辨证分为阳虚证、阴虚证、血瘀证、血热证、气虚证5个证型,检测各证型患者血淋巴细胞亚群包括CD3^+、CD4^+、CD8^+、CD4^+/CD8^+、CD19^+、CD16^+56^+的表达情况,分析各证型患者淋巴细胞亚群表达的差异性。结果 186例ITP患者气虚证80例、阴虚证57例、阳虚证24例、血热证15例、血瘀证10例。CD8^+在5个基本证型的表达差异有统计学意义(P<0.05),其余各指标在5个证型表达差异无统计学意义(P>0.05)。CD3^+表达在阳虚证与阴虚证之间差异有统计学意义,CD4^+表达在阴虚证与气虚证之间差异有统计学意义,CD8^+表达在阳虚证与血瘀证、阳虚证与血热证、阳虚证与气虚证、气虚证与阴虚证之间差异均有统计学意义,CD4^+/CD8^+阴虚证与血瘀证、阴虚证与气虚证之间差异均有统计学意义(P<0.05);CD19^+和CD16^+56^+表达在5个证型之间两两比较差异均无统计学意义(P>0.05)。结论 CD8^+在ITP不同证型患者的表达均有差异,而CD4^+、CD8^+、CD4^+/CD8^+在阴虚证与气虚证之间的表达均有差异。Objective To compare the difference of expression of lymphocyte subsets in patients with immune thrombocytopenia（ ITP） of different syndrome. Methods Collected 186 patients with ITP and divided them into 5syndrome types,including Yang deficiency,Yin deficiency,blood stasis,blood heat and Qi deficiency syndromes.Through the blood samples of patients with each syndrome type,examined the expression of lymphocyte subsets,including CD3＋,CD4＋,CD8＋,CD4＋/CD8＋,CD19＋and CD16＋56＋. Analyzed expression difference of lymphoid subsets cells in patients with different syndrome. Results In 186 patients with ITP,there were 80 cases of Qi deficiency syndrome,57 cases of Yin deficiency syndrome,24 cases of Yang deficiency syndrome,15 cases of blood heat syndrome and 10 cases of blood stasis syndrome. The differences in expression of CD8＋among the 5 basic syndrome types were statistically significant（ P 0. 05）,and the differences of the other indices among 5 syndromes were not statistically significant（ P 0. 05）. The expression difference of CD3＋between Yang deficiency and Yin deficiency syndrome was statistically significant. The difference of CD4＋expression between Yin deficiency syndrome and Qi deficiency syndrome was statistically significant. There was statistically significant difference between the expression of CD8＋in the Yang deficiency and blood stasis syndrome,Yang deficiency and blood heat syndrome,Yang deficiency and Qi deficiency syndrome,Qi deficiency and Yin deficiency syndrome. The differences of CD4＋/CD8＋between Yin deficiency syndrome and blood stasis syndrome,Yin deficiency and Qi deficiency syndrome were statistically significant（ P 0. 05）. The differences of expression of CD19＋and CD16＋56＋among the 5 syndrome types were not statistically significant in pairwise comparison（ P 0. 05）.
