外周血单核细胞Toll样受体2及Toll样受体4表达与糖尿病肾病的相关性

摘　　要：目的检测糖尿病慢性肾病中外周血单核细胞Toll样受体2(TLR2)及Toll样受体4(TLR4)的表达水平,探讨TLR2、TLR4与糖尿病肾病的相关性。方法选取内分泌科住院的2型糖尿病患者152例,将患者分为单纯糖尿病组、早期糖尿病肾病组、临床糖尿病肾病组,另选取体检科健康体检者36例作为对照组。采用流式细胞仪检测TLR2和TLR4的蛋白表达。采用RT-PCR检测TLR2和TLR4mRNA的表达,采用ELISA检测血清MCP-1、hs-CRP、TNF-α、IL-6和IL-18水平。结果单纯糖尿病组、早期糖尿病肾病组TLR4 mRNA、TLR4蛋白、TLR2 mRNA和TLR2蛋白水平均低于临床糖尿病肾病组(P<0.01);单纯糖尿病组、早期糖尿病肾病组MCP-1、hs-CRP、TNF-α、IL-6和IL-18水平均低于临床糖尿病肾病组(P<0.01);TLR2水平与UAE呈正相关(Y=0.013 X+1.295 6,R2=0.895 5),TLR4水平与UAE呈正相关(Y=0.006 X+1.650 0,R2=0.802 6)。结论 TLR4和TLR2水平可以反映糖尿病肾病发展程度,TLR4和TLR2可作为糖尿病肾病的检测指标。Objective To detect the expression levels of Toll like receptor 2（TLR2）and Toll like receptor 4（TLR4）in peripheral blood mononuclear cells in diabetic nephropathy,and to investigate the correlation between TLR2 and TLR4 with chronic nephropathy.Methods One hundred and fifty-two inpatients with T2 DM in the endocrinology department of our hospital were selected and divided into the simple diabetes mellitus group（SDM）,early diabetic nephropathy group（EDN）and clinical diabetic kidney disease group（DKD）.Other 36 individuals undergoing the physical examination were selected as the control group.The flow cytometry was used to detect the protein expression of TLR2 and TLR4.The expression of TLR2 and TLR4mRNA was detected by RTPCR,and the serum levels of MCP-1,hs-CRP,TNF-α,IL-6and IL-18 were detected by ELISA.Results The levels of TLR4 mRNA,TLR4protein,TLR2 mRNA and TLR2 protein in the SDM group and EDN group were significantly lower than those in the DKD group（P〈0.01）;the levels of MCP-1,hs-CRP,TNF-α,IL-6and IL-18 in the SDM group and EDN group were significantly lower than those in the DKD group（P〈0.01）;TLR2and UAE showed a positive correlation（Y =0.013 X＋1.295 6,R2=0.895 5）,TLR4 was positively correlated with UAE（Y=0.006 X＋1.650 0,R2=0.802 6）.Conclusion TLR4 and TLR2levels can reflect the development degree of diabetic nephropathy,so TLR4 and TLR2can be used as the detection indexes of diabetic nephropathy.
