维生素D_3水平与原发性干燥综合征的相关性研究

摘　　要：目的研究维生素D_3水平与原发性干燥综合征的相关性。方法选取2012年2月—2016年2月收治的84例原发性干燥综合征患者作为观察组,另选正常体检的健康体检者42例作为对照组。按照干燥综合征疾病活动指数(SSDAI)积分再将观察组分为活动组(SSDAI≥5分)和稳定组(SSDAI<5分),每组42例。采用ELISA检测血清中1,25-二羟基维生素D_3[1,25(OH)_2D_3]、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、IL-10、IL-17的表达情况;采用流式细胞仪检测外周血CD27^(high)细胞、CD27^+记忆性B细胞的表达水平。结果观察组1,25(OH)_2D_3水平明显低于对照组(P<0.05)。活动组1,25(OH)_2D_3水平低于稳定组和对照组(P<0.05)。观察组CD27^(high)细胞、CD27^+记忆性B细胞和IL-10水平均低于对照组(P<0.05),TNF-α、IL-6和IL-17水平均高于对照组(P<0.05)。活动组CD27^(high)细胞、CD27^+记忆性B细胞和IL-10水平均低于稳定组和对照组(P<0.05),稳定组低于对照组(P<0.05);活动组患者TNF-α、IL-6和IL-17水平均高于稳定组和对照组(P<0.05),稳定组高于对照组(P<0.05)。1,25(OH)_2D_3与CD27^(high)细胞、CD27^+记忆性B细胞和IL-10水平呈正相关(P<0.05),与TNF-α、IL-6及IL-17水平呈负相关(P<0.05)。结论维生素D_3的表达可能参与原发性干燥综合征的发病,并且与原发性干燥综合征的严重程度及免疫功能有关。Objective To study the correlation between plasma vitamin D3 levels and primary sjogren＇s syn- drome. Methods A total of 84 patients with primary sjogren＇s syndrome admitted during February 2012 and February 2016 were selected as observation group, and 42 healthy people taking medical examination were selected as control group. According to the Sjogreng syndrome disease activity index （SSDAI） scores, the observation group were divided in- to active subgroup （SSDAI ≥ 5, n = 42） and stable subgroup （SSDAI 〈 5, n = 42 ）. Expression levels of serum 1,25- two hydroxy vitamin D3 [ 1,25 （OH） 2 D3 ] , interleukin-6 （IL-6） , tumor necrosis factor-α （TNF-α） , IL-10 and IL-17 were detected by enzyme-linked immunosorbent assay （ELISA） method. Expression levels of CD27high and CD27 ＋ memory B cells in peripheral blood were detected by flow cytometry. Results In observation group, 1,25（OH）2 D3 level was sig- nificantly lower than that in control group （P 〈 0.05）. In active subgroup, 1,25 （OH）2 D3 level was significantly lower than those in stable subgroup and control group （P 〈 0.05 ）. In observation group, levels of CD27high cells, CD27 memory B ceils and IL-10 were significantly lower （ P 〈 0.05 ） , while levels of TNF-α, IL-6 and IL-17 were significantly higher than those in control group （P 〈0. 05）. Levels of CD27high cells, CD27＋ memory B cells and IL-IO in active sub- group were significantly lower than those in stable subgroup and control group （P 〈 0.05） , and the levels in stable sub- group were significantly lower than those in control group （P 〈0.05）. Levels of TNF-c~, IL-6 and IL-17 in active sub- group were significantly higher than those in stable subgroup and control group （P 〈 0.05 ） , and the levels in stable sub- group were significantly higher than those in control group （ P 〈 0.05 ）. The 1,25 （OH） 2 D3 was positively correlated with CD27high cells, CD27 ＋ memory B cells and IL-10 levels （P 〈 0. 05�
