摘 要:目的探讨瘦素、肥胖基因受体(OB-R)和白介素-β(IL-β)在病理性瘢痕组织中的表达水平及临床意义。方法收集2010年10月—2015年9月整形外科的110例组织标本,分为病理性瘢痕组50例(包括瘢痕疙瘩组27例和增生性瘢痕组23例)、非病理性瘢痕组30例和正常皮肤组30例。并用免疫组织化学方法测定,且比较各组瘦素、OB-R和IL-β的表达水平,并分析其临床意义。结果病理性瘢痕组和非病理性瘢痕组的瘦素、OB-R和IL-β表达水平均高于正常皮肤组(P<0.01),且病理性瘢痕组高于非病理性瘢痕组(P<0.01)。瘢痕疙瘩组瘦素、OB-R和IL-β表达水平高于增生性瘢痕组及非病理性瘢痕组(P<0.05),且增生性瘢痕组高于非病理性瘢痕组(P<0.05)。结论瘦素、OB-R和IL-β在瘢痕组织中表达异常增多,而其表达水平与瘢痕的纤维化程度相关,表明瘦素、OB-R和IL-β参与了病理性瘢痕组织的病变过程。Objective To investigate expressions and clinical significances of leptin, obese gene receptor (OB- R) and interleukin-β (IL-β) in pathological scar tissues. Methods A total of 110 tissue specimens in plastic surgery department during October 2010 and September 2015 were collected, and were divided into pathological sear group (n = 50, including 27 cases of keloid subgroup and 23 cases of hypertrophic scar subgroup) , non-pathological scar group (n = 30) and normal skin group (n = 30). Leptin, OB-R and IL-β expressions were detected and compared in all groups by immunohistoehemical staining method, and clinical significances were analyzed. Results Leptin, OB-R and IL-β expressions in pathological scar and non-pathological scar groups were significantly higher than those in normal skin group (P 〈 0. 01 ) , and the expressions in pathological scar group were significantly higher than those in non-pathological scar group (P 〈 0. 01 ). Leptin, OB-R and IL-β expressions in keloid subgroup were significantly higher than those in hyper- trophic scar subgroup and non-pathological scar group ( P 〈 0.05 ) , and the expressions in hypertrophic scar subgroup were significantly higher than those in non-pathological scar group (P 〈 0. 05). Conclusion Leptin, OB-R and IL-β expressions are increased abnormally in scar tissues, and the expressions are related to fibrosis degree of scar, which indicates that leptin, OB-R and IL-β involve in the pathological changes of pathological scar tissues.
