神经导航在幕上脑胶质瘤手术中的应用

摘　　要：目的:介绍神经导航系统在切除幕上脑胶质瘤手术中的初步应用体会。方法:应用上海复旦数字医疗科技有限公司生产的excelim04导航系统辅助切除幕上脑胶质瘤32例,并对神经导航用于脑胶质瘤手术的疗效、优越性及其注意事项进行分析。结果:在本组病例中,神经导航平均注册误差为(1.2±0.5)mm,术后72小时内复查MRI证实肿瘤影像学全切除率为85.7%,所有患者临床症状得到改善,肢体活动等重要神经功能较术前好转,无手术并发症,短期随访期间2例复发。结论:神经导航系统对于切除脑胶质瘤具有定位准确、动态示踪、微侵袭等特点,有助于提高胶质瘤全切除率,降低手术并发症。Objective：To introduce the application of neuronavigation system in the resection of glioma. Methods：We used the excelim04 navigation system in 32 patients with Supratentorial gliomas.The value of this system for glioma resection was assessed,and the advantages and cautions for application were discussed.Results：Of our cases,the mean registration error was 1.2＋0.5mm. Within 72 hours after operation, the total resection rate of tumor imaging was confirmed by MRI scan at the rate of 85.7% .The clinic status of all patients showed improvement,and the critical funetions,eg motor,were not significantly affected.There were no complications attributed to the use of this system.Only two patients relapsed during the short-time followup.Conclusion：Neuronavigation system has the characteristics of accurate positioning, dynamic tracing, micro invasion and so on, which can improve the total resection rate and reduce the complications.
