摘 要:目的观察血小板微颗粒(PMP)、血管性血友病因子(v WF)、纤维连接蛋白(FN)在血小板减少的血液病患者中与出血程度的关系,探讨PMP、v WF、FN对出血程度预测的临床意义。方法根据WHO关于出血的分级标准,将患者分为0分组、1分组、2分组、3分组、4分组及对照组。应用流式细胞术检测患者外周血中PMP含量变化,应用ELISA法检测患者外周血中v WF、FN含量变化。结果 (1)各组PMP、v WF、FN的含量差异均有统计学意义(P<0.001);(2)不同出血程度组之间出血程度越重,PMP含量越低[除1分组与2分组;3分组、4分组和对照组差异无统计学意义,其余差异有统计学意义(P<0.001)]、VWF含量越高[除1分组与2分组外,其余各组之间差异有统计学意义(P<0.001)]、FN含量越高[除0分组与1分组外,其余各组均差异有统计学意义(P<0.001)]。结论 PMP、v WF、FN的含量对血小板减少的血液病患者出血程度的预测具有临床意义。Objective To measure the relationship between the degree of bleeding and platelet microparticles(PMP),von Willebrand factor(v WF),fibronecti(FN) in hematologic patients with low platelet count,analyze the clinical significance of PMP,v WF,FN and perdict the degree of bleeding. Methods According to WHO grading standards of the degree of bleeding,the patients were divided into 0 group,1 group,2 group,3 group,4 group and health group.Flow cytometry was used to detect PMP in the peripheral blood in the patients.The enzyme-linked immunosorbent assay was used to determine the v WF and FN in the peripheral blood in the patients. Results(1)The level of PMP,v WF and FN had significance difference in each group( P〈0. 001).(2) In the different degree of bleeding,severity bleeding with much lower PMP[1 group and 2 group;3 group,4 group and the control group had no significance difference,others had significance difference( P〈0. 001)],much high v WF[each group had significance difference( P〈0. 001),except 1 group and 2 group],much higher FN[each group had significance difference( P〈0. 001),except 0 group and 1 group].Conclusion The level of PMP,v WF,FN is helpful to perdict the degree of bleeding in hematologic patients with low platelet count.
