中西医结合治疗更年期抑郁症临床研究

摘　　要：目的:观察中西医结合治疗更年期抑郁症肾虚肝郁证的临床疗效及对血清中5-羟色胺(5-hydroxy tryptamine,5-HT)和去甲肾上腺素(norepinephrine,NE)水平的影响。方法:将62例患者随机分为对照组和观察组,每组31例。对照组采取氟哌噻吨美利曲辛片治疗,每天2片,早晨及中午各1片。观察组在对照组基础上给予滋水清肝饮加减治疗,每天1剂,分两次温服。两组患者均连续治疗8周。治疗后比较两组患者抑郁自评量表(self-rating depression scale,SDS)、汉密尔顿抑郁量表(Hamilton depression scale,HAMD)-17评分及临床疗效。检测两组患者血清中5-HT和NE水平。结果:治疗后4周和8周,治疗组SDS和HAMD评分明显少于对照组,差异有统计学意义(P<0.01)。观察组有效率为90.32%,明显高于对照组64.52%,两组比较,差异有统计学意义(P<0.05)。治疗后观察组血清中5-HT和NE水平明显高于对照组,差异有统计学意义(P<0.01)。结论:中西医结合治疗更年期抑郁症肾虚肝郁证疗效显著,上调患者血清中5-HT和NE水平可能与其作用机制有关。Objective：To investigate of efficacy of combination of Chinese traditional and Western medicine in treating menopausal depression with kidney deficiency and liver qi stagnation syndrome and the influence on serum levels of 5-hydroxytryptamine（5-HT） and norepinephrine（ NE）. Methods：66 cases with menopausal depression were randomly divided into control group（ 31 cases） and observation group（31 cases）. The control group was orally given Flupentixol and Melitracen,2 tablet a day,separately in morning and evening. Based on control group,the observation group was treated with Zishui Qinggan Decoction,1 dose a day,twice in warm decoction. Both groups was treated for 8 weeks. Scores of SDS and HAMD-17 and efficacy were compared in both groups.After treatment,serum levels of 5-HT and NE were detected between both groups. Results：After 4 and 8 weeks＇ treatment,scores of SDS and HAMD-17 of the observation group were evidently lower than control group（ P〈0. 01）. Efficacy of the observation group was 90. 32%,which was remarkably higher than control group 64. 52%（ P〈0. 05）. Serum levels of 5-HT and NE were higher than those of control group with statistical significance after treatment（ P〈0. 01）. Conclusion：Efficacy of combination of traditional Chinese and Western medicine in treating menopausal depression is obvious,which may be related to up-regulation of serum levels of 5-HTand NE.
