改良天灸散穴位贴敷虚寒证老年腰痛的临床研究

摘　　要：目的:观察虚寒证老年腰痛患者在不同配方天灸散(传统天灸散和改良天灸散)及不同调敷方式(姜汁和蜂蜜)穴位贴敷治疗后的治疗效果,为虚寒证老年腰痛患者的康复治疗提供指导。方法:将120例虚寒证老年腰痛患者随机分为A组、B组、C组各40例,A组采用改良天灸散配方药粉,药粉采用传统生姜汁调敷;B组采用改良天灸散配方药粉,药粉采用蜂蜜调敷;C组(对照组)采用传统天灸散穴位贴敷,每日贴敷1次,每次2小时,10次为1个疗程,共治疗3个疗程,疗程间休息20天,采用虚寒证辨证因子等级评定量表和视觉模拟疼痛评分(VAS)量化评估临床疗效,并观察三组患者皮肤损伤情况。结果:经治疗后,A组、B组虚寒证辨证因子等级评分、VAS评分改善程度均显著优于C组(P<0.05),A组、B组间差异则无统计学意义(P>0.05);三组皮肤损伤率为B组<A组<C组,相互比较差异均有统计学意义(P<0.05)。结论:改良天灸散穴位贴敷虚寒证老年腰痛病疗效确切,蜂蜜调敷相比姜汁调敷具有皮肤损伤率低的临床优势;改良天灸散蜂蜜调敷穴位贴敷能较好解决传统天灸散敷贴疗法对老年腰痛患者治疗时产生皮肤损伤的普遍性问题;改良天灸散穴位贴敷虚寒证老年腰痛具有疗效确切、安全性较高、老年患者治疗产生皮肤损伤少等临床应用优势。Objective： To observe the efficacy of acupoints application with different formulations of medicinal vesiculation pulvis （conventional and modified medicinal vesiculation pulvis） and different supplementary materials （ginger juice and honey） for elderly patients with low back pain of as- thenia cold syndrome, in order to provide guidance for their rehabilitation treatment. Methods： 120 cases of elderly patients with low back pain ofas- thenia cold syndrome were randomly divided into group A （40 cases）, group B （40 cases） and group C （40 cases）, group A used modified medicinal vesiculation pulvis which was mixed with ginger juice while group B was honey, and group C （control group） used conventional medicinal vesicula- tion pulvis, the treatment was performed daily and 2 hours every time, 10 times as one course of treatment and for a total of 3 with 20 days off be- tween courses, then used Rating Scale for Syndrome Differentiation Factors of Asthenia Cold Syndrome and Visual Analogue Scale （VAS） to quanti- tatively evaluate efficacy and observed the skin lesions of the three groups. Results： After treatment, the improvement of rating score for syndrome differentiation factors of asthenia cold syndrome and VAS score of group A and B were significantly better than those of group C （P〈0.05）, while dif- ferences had no statistical significance between group A and B （P〉0.05）; the rate of skin lesions was： group B〈group A〈group C, which had signifi- cant differences among the groups （P〈0.05）. Conclusion： Modified medicinal vesiculation pulvis has a definite curative effect on low back pain ofas- thenia cold syndrome in elderly patients, and mixed with honey has a lower rate of skin lesions than ginger juice; acupoints application with modified medicinal vesiculation pulvis mixed with honey can solve the universality problem of skin lesions due to the conventional medicinal vesiculation pul- vis in treatment of low back pain in elderly patients; acupoints applic
