骨通贴膏联合神经损伤治疗仪、颈椎推拿及牵引治疗神经根型颈椎病临床研究

摘　　要：目的观察骨通贴膏联合神经损伤治疗仪、颈椎推拿及牵引治疗神经根型颈椎病(CSR)的临床疗效。方法将54例CSR患者随机分为2组,对照组27例行布洛芬凝胶联合颈椎推拿、牵引治疗,治疗组27例行骨通贴膏联合神经损伤治疗仪、颈椎推拿及牵引治疗。2组均治疗16 d后应用简化Mc Gill疼痛评分表(SF-MPQ)评价2组上肢放射痛改善情况,采用田中靖久症状量化表20分法评价颈椎病综合症状和体征改善情况,检测2组治疗前后体感诱发电位(SEP)P40峰潜伏期,统计2组临床疗效及症状消失时间。结果2组治疗后SF-MPQ各项评分均较本组治疗前降低,症状量化表20分法评分升高(P<0.05),且2组组间比较差异均有统计学意义(P<0.05)。治疗后治疗组SEP P40峰潜伏期较治疗前显著缩短(P<0.05),且短于对照组(P<0.05);对照组治疗前后SEP P40峰潜伏期无明显变化(P>0.05)。治疗组总有效率92.59%,对照组70.37%,2组总有效率比较差异有统计学意义(P<0.05),治疗组临床疗效优于对照组,且症状消失时间早于对照组(P<0.05)。结论骨通贴膏联合神经损伤治疗仪、颈椎推拿及牵引治疗CSR疗效确切,可更有效改善患者临床症状,促进神经根功能恢复。Objective To observe the clinical effects of Gutong plaster combined with Miriam nerve injury treatment,Tuina,traction and physical therapy on cervical spondylotic radiculopathy（ CSR）. Methods 54 CSR patients were randomly divided into two groups. 27 cases in control group were treated by Ibuprofen gel combined with Tuina,traction and physical therapy of cervical vertebra. 27 cases in treatment group were treated by Gutong plaster combined with Miriam nerve injury treatment,Tuina,traction and physical therapy of cervical vertebra. The improvement of upper limb radiating pain was evaluated by Simplified Mc Gill Pain Question（ SF-MPQ） after 16 weeks treatment. The symptomes complice and signs of CSR was evaluated by Tanaka Yasuku symptom quantization table with 20 points. The P40 latency of somatosensory evoked potential（ SEP） was measured before and after treatment,and the clinical effects and symptom disappearance time were observed. Results The SF-MPQ after treatment were decreased in two groups,the score of Tanaka Yasuku symptom quantization table with 20 points after treatment was increased（ P〈0. 05）,and improvements in treatment group was superior to control group（ P〈0. 05）. The P40 latency of SEP after treatment was significantly decreased in treatment group（ P〈0. 05）,which was shorter than control group at the same period（ P〈0. 05）,and there was on obviously changes of P40 latency between before and after treatment in control group（ P〉0. 05）. There was significant difference on total effective rate between treatment group（ 92. 59%） and control group（ 70. 37%,P〈0. 05）,the curative effects in treatment group was superior to control group,and the symptom disappearance time was earlier than control group（ P〈0. 05）. Conclusion Gutong plaster combined with Miriam nerve injury treatment,Tuina,traction and physical therapy have exact on CSR,can effectively improve the clinical symptoms of CSR,promote the recovery of nerve root.
