深圳市公立综合性医院首诊测血压信息平台建设的现况分析

摘　　要：目的掌握公立综合性医院首诊测血压信息平台建设现况,科学评价已建平台实用性及存在问题,为今后深圳及其他地区医疗机构首诊测血压信息平台的完善和建设提供科学依据和借鉴。方法 2016年7月份,对深圳市所有48家公立综合性医院开展首诊测血压专题调查。依据首诊测血压的工作目标和信息化平台建设标准,制定《医疗机构首诊测血压信息平台评分表》,评价各家医院平台实用性。采用SPSS 16.0统计软件对首诊测血压信息平台建设情况、综合评分、性能评分和性能实现情况进行统计描述和统计推断。结果深圳市公立综合医院已建成首诊测血压信息平台38家,建设率79.2%(38/48),其中,市、区属医院建设率分别为44.4%(4/9)和87.2%(34/39)(χ~2=5.713,P=0.017)。已建成的首诊测血压信息平台中,最低综合评分为10分,最高综合评分为88分,84.2%(32/38)综合评分<60分。平台性能评分,医疗机构间的互联互通得分最低,仅0.79分,其次为护士分诊平台的血压核实和血压平行录入,分别为1.32和1.58分,单位内部互联互通、统计分析、患者识别及医生平台的血压核实也均不足6分。性能实现率方面:可统计高血压确诊数最低,仅5.3%(2/38);其次为医疗机构间的互联互通,仅7.9%(3/38);与护士平台相关的血压核实、血压录入及互联互通实现率均较低,分别为13.2%(5/38)、15.8%(6/38)和21.1%(8/38);对首诊患者的识别和统计亦很低,为15.8%(6/38)。结论深圳市首诊测血压信息平台建设进度和水平参差不一,信息平台尚普遍不完善,需逐一完善各性能,尤其互联互通、护士平台和首诊患者识别方面。Objective To master the current situation of the information platform construction of Office Blood Pressure Monitoring for First-time Visits （OBPMFV） in the public general hospitals in Shenzhen, evaluate the practicability of the platforms, and provide reference for the improvement and construction of the information platforms of OBPMFV in Shenzhen and other areas in the future. Methods In July 2016, a special investigation of OBPMFV was carried out in all 48 public general hospitals in Shenzhen. According to the target and information platform construction standards of OBPMFV, the rating scale was designed, and the practicability of the information platforms was evaluated. Statistical description and inference were undertaken to analyze the scores and performance of the platforms by SPSS 16.0 statistical software. Results 79.2% （38/48） public general hospitals had built the information platform of OBPMFV in Shenzhen. The construction rates of the municipal and district hospitals were 44.4% （4/9） and 87.2% （34/39）（χ^2 =5. 713, P=0. 017）, respectively. Among 38 platforms, 84.2% （32/38） information platform scored below 60, with the lowest of 10, and highest of 88. On performance score, the score of interconnection between medical institutions was the lowest, only 0.79, followed by blood pressure check of nurse triage platform and parallel entry （1.32 and 1.58, respectively）. Scores of interconnection within hospital, statistical analysis, patient identification and blood pressure check of doctor platform were also less than 6. Among per formance implementation, the rate of the capacity for counting up the number of diagnosed hypertension was the lowest, only 5.3% （2/38）, followed by the rate of interconnection between medical institutions, which was 7.9% （3/38） ; The implementation rates of blood pressure check, entry and interconnection related to nurse platform were also low,which were 13.2% （5/38）, 15.8% （6/38） and 21.1% （8/38） respectively; The rate of identific
