摘　　要：目的:探讨表观扩散系数(apparent diffusion coefficient,ADC)与直肠癌患者病理类型及预后指标的相关性。方法:对72例直肠癌患者进行回顾性分析,比较不同组别患者ADC值,并用受试者工作特征(receiver operating characteristic,ROC)曲线确定各因素的ADC鉴别界值。结果:不同T分期的患者ADC值有显著差异,淋巴结外肿瘤种植阳性患者ADC值明显低于阴性患者,术前糖类抗原19-9(carbohydrate antigen19-9,CA19-9)≥35 g/m L的患者ADC值明显低于CA19-9<35 g/mL的患者。ADC值对T2/T3~4、淋巴外肿瘤种植及术前CA19-9水平的鉴别界值分别为1.215×10^(-3)、1.335×10^(-3)和1.165×10-3 mm^2/s。结论:ADC值与直肠癌预后指标相关性较强,具有一定的预测价值,有望成为一种无创的直肠癌预后生物学标志物。Objective： To analyze the correlation between pre-treatment apparent diffusion coefficient （ADC） and pathological pattern and prognostic factors of rectal cancer. Methods： The data of 72 rectal cancer patients were retrospectively analyzed, and the ADC values were compared. The ADC cut-off value for each prognosis factor was determined with receiver operating characteristic （ROC） curve. Results： The ADC values of different T stages were statistically significant. The ADC values of tumors with extranodal tumor deposits or carbohydrate antigenl9-9 （CA19-9） level ≥ 35 g/mL were significantly lower. The ADC cut-off values for T2/T3-4, extranodal tumor deposits and preoperative CA19-9 level were 1.215× 10^-3, 1.335×10^-3 and 1.165×10^-3 mm2/s, respectively. Conclusion： The ADC value of rectal cancer is strongly associated with the prognostic factors of rectal cancer, which indicates that the ADC value is a potential new noninvasive biomarker for rectal cancer prognosis.
