蒋位庄教授辨证分型诊治椎间盘源性腰腿痛的哲学思想

摘　　要：椎间盘源性腰腿疼,是指始发于椎间盘的损伤、破裂、突出而引起腰痛伴下肢放射性疼痛,以腰痛伴下肢放射性疼痛为主症。蒋位庄教授为中国中医科学院望京医院主任医师,教授,博士生导师,享受国务院政府特殊津贴。从事骨伤科临床医疗、科研、教学50余年,曾任中华医学会疼痛学会副主任委员、常委、兼腰背痛学组组长。蒋位庄教授等从20世纪80年代开始,结合中医古籍及自己多年的临床实践,运用中医哲学思维,以中医整体观为依据,提出了"椎间盘源性腰腿痛"的观念。在辨证论治原则的指导下,根据椎间盘各组成部分及其相关组织不同阶段的病理生理变化,从椎间盘到小关节、从青年到中老年的病情发生发展过程,提出了互为因果关系的分型诊断及相应的治疗原则。这一哲学思维的创造性的应用,加深了我们在腰腿痛方面的中医理论认识,对中西医结合治疗腰腿痛有实际应用价值。Intervertebral disc origins waist and leg pain, refers to the origin of the disc injury, rupture, protrusion caused by lumbar spinal pain associated with lower extremity, with low back pain associated with lower limb radiation pain. Professor Jiang Weizhuang in Wangjing Hospital of Chinese Academy of Traditional Chinese Medicine, Chief Physician, Professor, doctoral tutor, enjoys special government allowances of the State Council, has been engaged in orthopedic clinical medicine, scientific research, teaching more than50 years, and served as vice chairman of the Chinese Medical Association pain, member of the Standing Committee, and back pain group leader. From the 1980 s, combined with ancient Chinese medicine and their own years of clinical practice, use of traditional Chinese medicine philosophy, the overall view of traditional Chinese medicine as the basis, Professor Jiang Weizhuang proposed ＂discogenic low back pain＂ concept, according to the principle of syndrome differentiation and treatment, according to the pathophys-iological changes of intervertebral discs and related tissues at different stages, from the disc to the facet joints, from young to middle-aged patients, the classification of diagnosis and treatment and the corresponding treatment principles. The creative application of this philosophical thinking has deepened our understanding of TCM theory in the treatment of lumbar and leg pain, and has practical val-ue in the treatment of lumbago and leg pain with integrated Chinese and Western medicine.
