摘　　要：目的:评价右美托咪啶(Dexmedetomidine,DEX)复合罗哌卡因(Ropivacaine,ROP)在超声引导下臂丛神经阻滞镇痛中的效果。方法:选入2014年5月至2016年5月在江西省宜丰县人民医院行上肢手术的患者60例,随机分为A、B两组,每组30例,均行超声引导下臂丛神经阻滞,其中A组使用DEX复合ROP,B组采用单纯ROP。比较两组的镇痛效果。结果:A组在术后6h、12h、24h和48h时的VAS评分显著低于B组(P<0.05);A组术后吗啡首次使用时间及48h用量均显著优于B组(P<0.05)。结论:超声引导下臂丛神经阻滞使用DEX复合ROP,可明显延长镇痛时间,推迟术后吗啡使用时间、减少48h吗啡用量。Objective：To evaluate the analgesic effect of DEX combined with ROP in ultrasound guided brachial plexus nerve block.Methods：60 patients,who underwent upper extremity surgery from May 2014 to May 2016,were randomly divided into group A and B,with 30 cases in each group.All were given ultrasound guided brachial plexus nerve block,where a group of DEX composite ROP,group B with simple ROP.Analgesic effects of the two groups were compared.Group A were given DEX with ROP,group B with only ROP.Analgesic effects of the two groups were compared.Results：The VAS scores of 6 h,12 h,24 h and 48 h in A group were significantly lower than group B（P〈0.05）.The first time and the amount of 48 h in group B was significantly better than group A（P〈0.05）.Conclusion：DEX combined with ROP used in ultrasound guided brachial plexus block can significantly prolong the duration of analgesia,delay the use of morphine and reduce the dosage of 48 h.
