摘 要:目的探讨64排128层螺旋CT检查新生儿颅脑低剂量扫描方案的可行性。方法采用数字表法,将连续受检的123例新生儿(77例足月新生儿和46例早产儿)随机分为3组,分别行颅脑常规剂量及低剂量扫描,扫描管电压均为120kv,管电流分别为150mAs、40mAs及30mAs。比较3组CT吸收剂量指数(CTDIw)、剂量长度乘积(DLP)及图像质量差异。结果低剂量组X线吸收剂量明显减低。随管电流降低,3组被试图像质量逐渐降低(P<0.001),不合格图像逐渐增加,但40mAs组与常规组可满足诊断要求的图像数量比较,差异无统计学意义(P>0.05);30mAs组的足月新生儿不合格图像较多,而30mAs组和40mAs组可满足诊断要求的早产儿图像数量比较,差异无统计学意义(P>0.05)。结论 128层螺旋CT 40mAs及30mAs新生儿颅脑扫描可满足临床诊断需求,能有效降低新生儿的辐射剂量,40mAs及30mAs扫描法可分别作为足月儿及早产儿颅脑CT检查的首选方案。Objective To investigate the optimal protocol of low-dose CT imaging in the neonatal cerebral examination with 128-slice spiral CT.Methods 123 newborns including 77 mature infants and 46 premature infants were randomly divided into 3groups,which are the standard-dose groups with the tube current of 150 mA,the lowdose group with 40 mA,and the low-dose group of 30 mA.All the three groups underwent the same tube voltage of129 KV.The weighted CT dose index(CTDIw),the dose-length product(DLP)and the image quality were compared among the three groups.Results The X-ray absorbed doses in the low-dose groups were significantly reduced.With the decrease of the tube current,the image quality became significantly worse(P〈0.001)and the unqualified images increased,but there was no significant differences in the image numbers of meeting the diagnosing demands between in the low-dose group with 40 mA and the standard-dose group with 150 mA(P〉0.05).There were more unqualified images of mature infants in the low-dose group of 30 mA,but the numbers of premature infants′images of meeting the diagnosing demands were not significantly different between in the lowdose groups with 40 mA and with 30mA(P〉0.05).Conclusion The 128-slice spiral CT with the tube current of30 or 40 mA in the neonatal brains can meet the demands of clinical diagnosis and reduce the radiation dose of newborns.Therefore,the scanning methods with the tube current of 30 and 40mA can be selected as the optimal protocol of CT imaging in the brains of premature infants and mature infants respectively.
