摘 要:目的探讨丙型肝炎病毒(HCV)感染者血液中HCV RNA载量、HCV抗体及丙氨酸氨基转移酶(ALT)检测结果的相互关系。方法经ELISA法检测的HCV抗体阳性血样,采用胶体金法检测HCV抗体,同时用荧光RT-PCR检测血清HCV RNA载量,用全自动生化仪定量检测ALT。结果 255份ELISA检测为HCV抗体阳性样本中,154份胶体金法阳性,139份HCV RNA阳性,59份ALT结果异常。HCV抗体高值组(S/CO值≥10)中HCV RNA阳性率为85.49%,胶体金阳性率为96.95%,ALT异常率35.88%,均显著高于S/CO低值组(1<S/CO<5)和S/CO中值组(5≤S/CO<10)。胶体金阳性样本,HCV RNA阳性率(74.03%)显著高于胶体金阴性样本的HCV RNA阳性率(24.75%),二者差异有统计学意义(P<0.05)。胶体金阳性样本的ALT异常率(32.47%)显著高于胶体金阴性样本的ALT异常率(8.91%),二者差异有统计学意义(P<0.05)。HCV RNA阳性者的ALT异常率(35.50%)显著高于HCV RNA阴性者的ALT异常率(8.57%),二者差异有统计学意义(P<0.05)。ALT异常率随着HCV RNA载量的增高而升高,有一定相关性(r=0.975,P=0.001),而ALT变化与HCV RNA载量无相关性(r=-0.26,P=0.63)。结论对人群HCV感染的筛查,可在ELISA法检测HCV抗体基础上,对S/CO高值人群进行HCV RNA的检测,提高检出率的同时最大程度地降低成本。Objective To investigate the relationship among the HCV RNA,anti-HCV and ALT levels. Methods The serum samples of anti-HCV positive tested by ELISA were re-checked by colloidal gold method. Fluorescent quantitative RT-PCR was used to detect viral loads and kinetic method was employed to assay ALT levels. Results Among 255 anti-HCV positive samples,154 were positive for colloidal gold method,139 were positive for HCV RNA,59 were abnormal ALT level. The HCV RNA positive rate,colloidal gold positive rate and ALT abnormality rate in group with high S/CO ratios (S/CO〉~10) in anti-l-ICV antibody tests were 85.49%,96.95% and 35.88%,re- spectively with statistically differences to the group with low S/CO(I〈S/CO〈5) and the group with median S/CO(5≤ S/CO〈10). The HCV RNA positive rate and ALT abnormality rate were significantly higher in colloidal gold positive group than in colloidal gold negative group. The occurrence of elevated ALT levels in HCV RNA positive were more frequent than in HCV RNA negative. HCV RNA and ALT abnormal rates were correlated(r=0.975,P=0.001). ALT level and HCV RNA loads had no significant correlation (r=0.26,P=0.63). Conclusion It is recommended that serum HCV RNA detection should he performed in high anti-HCV S/CO ratio group in order to increase HCV de- tection rate with lower cost.
