腹腔镜下疝囊高位结扎结合脐正中襞覆盖疝内环口治疗婴幼儿复发性腹股沟斜疝的临床应用

摘　　要：目的观察腹腔镜疝囊高位结扎结合脐正中襞覆盖内环口治疗婴幼儿复发性腹股沟斜疝的效果。方法选取该院2014年1月‐2014年12月收治的33例复发性腹股沟斜疝患儿为研究对象,采用腹腔镜疝囊高位结扎结合脐正中襞覆盖内环口治疗,评价近期疗效。患者术后随访至2016年10月,记录复发情况。结果 33例患儿均于腹腔镜下完成手术,手术时间为(19.7±2.8)min;术中无1例患者出血及肠管损伤;术后住院时间为(3.1±0.9)d;术后无1例发生阴囊血肿、腹腔感染一级肠粘连等并发症,且近期痊愈率为100%;随访(28.9±4.2)个月,无1例复发。结论腹腔镜疝囊高位结扎结合脐正中襞覆盖疝内环口是治疗婴幼儿复发性腹股沟斜疝安全可行的方法,可有效避免复发。[Objective] To observe the effect of laparoscopic hernia sac high ligation combined with median umbilical fold covering the internal ring mouth for the treatment of infantile recurrent inguinal hernia. [Methods] Thirty-three cases of recurrent indirect inguinal hernia treated in our hospital from January 2014 to December 2014 were selected. They were treated with laparoscopic hernia sac high ligation combined with median umbilical fold covering the internal ring mouth, and the recent curative effect was evaluated. Patients were followed up until October 2016 after surgery, and the relapse was recorded. [Results] Laparoscopic surgery was complete in 33 cases, and the average operation time was （19.7 ± 2.8） rain; hemorrhage and intestinal canal injury occurred in 1 case; the average hospitalization time after surgery was （3.1 ± 0.9） d; no postoperative complications such as scrotal hematoma, abdominal infection, and intestinal adhesion were found, and the recent recovery rate was 100%, Average follow- up time was （28.9± 4.2） months, and no recurrence was observed. [Conclusion] Laparoscopic hernia sac high ligation combined with median umbilical fold covering hernia internal ring mouth is a safe and feasible method for infantile recurrent indirect inguinal hernia, which can effectively avoid the recurrence.
