钩板在老年锁骨远端粉碎性骨折中的应用

摘　　要：目的观察钛合金钩板内固定治疗老年性锁骨远端粉碎性骨折的疗效。方法 23例老年锁骨远端粉碎性骨折患者应用钛合金钩板治疗,同期修复损伤的喙锁韧带。术后1周起开始康复锻炼,观察骨折愈合时间、肩关节功能恢复情况。结果 23例患者术后均获随访,时间9~24个月。骨折复位良好,X线片显示锁骨骨折均愈合,愈合时间4~7个月;关节功能恢复良好,平均术后1 a取出内固定物,无关节再脱位。按照Karlsson标准评定术后肩关节功能,优良率95.6%。结论钛合金钩板治疗老年性锁骨远端粉碎性骨折疗效可靠。Objective To observe the outcome of senile comminuted fractures of distal clavicles treated by clavicle hook plate. Methods From September 2013 to September 2015,23 cases of senile comminuted fractures of distal clavicles were treated by open reduction and internal fixation with clavicle hook plates and coracoclavicularligment repairing. Rehabilitative training began 1 week after operation. Plain radiographs of clavicles were used to assess bony union. Functional recovery of the shoulder joint was assessed using the Karlsson standard. Results The 23 patients were followed up for 9 - 24 months（ average）. All fractures eventually achieved solid bony union from 4 to 7 months after surgery and no redislocation was observed. The hook plates were removed at an average of 12 months after the first operation.According to Karlsson shoulder joint function evaluation,the excellent and good rate was 95. 6%. Conclusion Open reduction and internal fixation with clavicle hook plate is a useful method for senile comminuted fractures of distal clavicles.
