甲泼尼龙冲击联合眶周注射曲安奈德治疗甲状腺相关性眼病临床研究

摘　　要：目的评价甲泼尼龙冲击治疗或联合曲安奈德眶周注射治疗甲状腺相关眼病的临床效果。方法自2013年2月-2016年8月,对28例双眼甲状腺相关眼病患者给予静滴甲泼尼龙每周1次,连续12周,前6周每次0.5g,后6周每次0.25g,同时对每例患者症状较重的一眼联合眶周注射曲安奈德,每次40mg,每3周一次,注射4次。结果患者双眼的炎症活动得到有效控制,眼球突出度和睑裂高度等症状改善,在联合治疗组效果更优,治疗前后差异均有统计学意义(P<0.05)。结论甲泼尼龙连续12周冲击法治疗甲状腺相关眼病有较好的疗效,联合眶周注射曲安奈德效果更佳。Objective Observe the efficacy of methylprednisolone pulse therapy combined with triamcinolone acetonide retrobul- bar injection in the treatment of thyroid associated ophthalmopathy. Methods 28 patients with thyroid associated ophthalmopathy in both eyes were included. Intravenous methylprednisolone were given once a week for 12 weeks, the dose was 0.5 g for the first 6 weeks and 0.25 g for the other 6 weeks. At the same time, retrobulbar injection of 40 mg triamcinolone acetonide were given to the more severe eye in every patient once every 3 weeks for 4 times in all. Results After 12 weeks treatment, the condition of inflammatory activity, exophthalmos and eyelid retraction were obviously improved. The efficacy of combined treatment group was better, and the differences were statistically significant. Conclusion Methylprednisolone pulse therapy for a continuous 12 weeks is significantly effective on thyroid associated ophthalmopathy, better efficacy can be achieved when combined with retrobulbar injection of triamcinolone acetonide.
