主要照顾者照护能力对膝关节置换患者围术期疼痛影响的纵向研究

摘　　要：目的:调查主要照顾者照护能力对全膝关节置换(TKA)患者围术期疼痛的影响,为术后疼痛循证护理提供依据。方法:采用数字评分法在术前1天,术日至术后第3天,术后1,3个月调查89例TKA患者自评疼痛程度;采用照护能力问卷在患者术前1天,出院前1天,术后1,3个月调查其主要照顾者照护能力。将不同时期照护能力得分分为低、中、高3组,使用广义估计方程分别分析3组主要照顾者在各个时间段所照顾患者的疼痛程度差异。结果:TKA患者围术期疼痛控制不理想,其主要照顾者照护能力在不同时期得分比较有显著统计学差异,照护能力高分组所照顾患者较中分和低分组围术期疼痛程度更高。结论:主要照顾者照护能力对患者围术期疼痛程度有显著影响,医疗机构在制定TKA术后疼痛管理政策时应充分考虑其主要照顾者照护能力。Objective：To investigate the influence of primary earegivers＇care ability on TKA patients＇perioperative pain, and to provide basis for postoperative pain evidence-based nursing. Methods： Digital scoring method was adopted one clay before surgery, on the operation day,3 days,1 month and 3 months after the operation to investigate 89 cases of postoperative TKA patients＇ self-reported pain degree. Care ability questionnaire was also adopted one day before surgery, 1 day before discharge, one month and 3 months after surgery to investigate the primary caregivers＇care ability. Care ability scores at different periods were divided into low, medium and high groups. Generalized estimating equations were respectively employed to analyze the pain intensity differences of the 3 groups in different periods of time. Results： The perioperative pain control of the TKA patients was not ideal. The care ability score comparison of the pri- mary caregivers in different periods had statistically significant difference. The pain degree of the patients in the high care ability group was higher in the perioperative period than those in the medium and low ability groups. Conclusion：Primary caregivers＇care ability has significant impact on perioperative pain degree. Medical institutions making TKA postoperative pain management policy should fully consider primary caregivers＇care ability.
