急性呼吸衰竭患者诊断中床旁超声的运用价值

摘　　要：目的:分析急性呼吸衰竭患者诊断中床旁超声快速诊断的运用效果。方法:选取笔者所在医院急诊抢救室及急诊ICU诊治的急性呼吸衰竭患者100例,按照不同诊断方法将其分成对照组与观察组,各50例,对照组运用常规评估方法进行诊断,观察组运用床旁超声快速诊断法进行诊断,对比两组患者的诊断与治疗时间,分析重症超声快速诊断的正确率、特异性及敏感性。结果:对比两组患者的初诊时间、确诊时间、初始正确治疗时间、其他诊断方法时间,观察组均明显短于对照组,差异有统计学意义(P<0.05);两组患者最终确诊的主要病因包括急性肺水肿、肺实变、慢性阻塞性肺疾病/哮喘、大量胸腔积液等,床旁超声快速诊断对这些病因的诊断大多具有较高的正确率、特异性及敏感性。结论:急性呼吸衰竭患者诊断中床旁超声快速诊断的运用效果显著,可准确评估患者病因,明显缩短其诊断时间与治疗时间,值得临床推广。Objective： To analyze the diagnostic efficiency of point-of-care uhrasonography for acute respiratory failure.Method： 100 patients with acute respiratory failure diagnosed and treated by emergency room and EICU were divided into control group and study group according to the diagnostic method. Conventional examination was applied to control group, while point-of-care ultrasound was added to study group.The time to preliminary diagnosis, time to final diagnosis, and time to effective treatment were documented and compared between two groups.Result： Compared with the control group, the observation time, initial diagnosis time, initial correct treatment time and other diagnostic methods of the two groups were significantly shorter than the control group, the difference was statistically significant（P〈0.05） ; the main causes of the two groups of patients eventually diagnosed including acute pulmonary edema, pulmonary consolidation, chronic obstructive pulmonary disease / asthma, pleural effusion, and the correct rate of bedside ultrasound diagnosis in diagnosis of these diseases have high specificity and sensitivity.Conclusion： Point-of-care ultrasound seems to be a promising approach to facilitate early diagnostic and therapeutic decision for patients with acute respiratory failure.
