输油管爆燃致骨折的影像表现

摘  要:目的分析输油管线爆燃致伤病人骨折部位及分型。方法回顾分析自2013年11月22—30日在我院放射科就诊的69例输油管爆燃致伤病人的影像学资料,其中行DR检查53例,投照部位132处;行CT检查60例,检查部位232处。结果 DR或CT检查55例病人为阳性,总阳性率约为79.71%,其中40例病人发生骨折,包括单部位骨折11例(27.50%),多部位骨折29例(72.50%),5例病人关节半脱位。本组病人共有13个部位发生了骨折,依次为腰椎骨折16例(40.00%),肋骨骨折13例(32.50%),胫腓骨骨折8例(20.00%),足踝骨骨折8例(20.00%),颅面骨骨折6例(16.00%),胸椎骨折6例(16.00%),骨盆骨折5例(12.50%),手腕骨骨折4例(10.00%),肩关节骨折3例(7.50%),尺桡骨骨折2例(5.00%),锁骨骨折2例(5.00%),肱骨骨折1例(2.50%),颈椎骨折1例(2.50%);按照骨折类型发生情况依次为斜行骨折25例(62.50%),压缩性骨折16例(40.00%),嵌插骨折13例(32.50%),粉碎性骨折5例(12.50%),横行骨折4例(10.00%),线形骨折4例(10.00%)),爆裂性骨折1例(2.50%),螺旋形骨折1例(2.50%)。结论爆燃致多部位骨折中腰椎骨折最常见,骨折类型多为斜行骨折。Objective To analyze the fracture type and site caused by oil pipeline deflagration. Methods Imaging data of 69 injured patiens-who came to X-ray Department of our hospital,during November 22-30,2013,for examination due to oil pipeline deflagration-were reviewed retrospectively.Among them,53 received an X-ray examination with 132 check points,and60 underwent a CT with 232 check points. Results Positive findings were noted in 55 patients,with a total positive rate of79.71%,of whom,40 with fractures-including 11(27.50%)with single site fracture,29(72.50%)with multiple site fracture,and five with subluxation of joint.Fractures occurred in 13sites:16(40.00%)of lumbar spine,followed by 13(32.50%)of ribs,eight(20.00%)of ibia and fibula,eight(20.00%)of ankle and foot bones,six(16.00%)of craniofacial bones,six(16.00%)of thoracic spine,five(12.50%)of pelvic,four(10.00%)of wrist and hand bones,three(7.50%)of shoulder joints,two(5.00%)of ulna and radius,two(5.00%)of clavicle,one(2.50%)of humerus and one(2.50%)of cervical vertebrae.According to the types of fracture,it was 25(62.50%)cases of oblique fracture,followed by 16(40.00%)of compression fracture,13(32.50%)of insert fracture,five(12.50%)of comminuted fracture,four(10.00%)of transverse fractur,four(10.00%)of linear fracture,one(2.50%)of blowout fracture and one(2.50%)of spiral fracture. Conclusion In multiple site fractures caused by deflagration,fracture of lumbar vertebra and oblique fracture are the most commonly seen.
