连续血液净化辅助甲基强的松龙及环磷酰胺联合冲击对狼疮性肾炎合并狼疮性肺炎患者Th1/Th2细胞因子失衡的影响

摘　　要：目的分析连续血液净化辅助甲基强的松龙及环磷酰胺联合冲击对狼疮性肾炎合并狼疮性肺炎患者Th1/Th2细胞因子失衡的影响。方法抽取2010年1月-2016年3月我院52例狼疮性肾炎合并狼疮性肺炎患者,随机数表法分为研究组(n=26)及对照组(n=26)。对照组采用甲基强的松龙+环磷酰胺联合冲击治疗,研究组采用连续血液净化辅助甲基强的松龙+环磷酰胺联合冲击治疗。对比两组临床症状改善情况及治疗前后Th1、Th2、Th1/Th2水平和干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、IFN-γ/IL-4水平变化情况,统计两组临床疗效。结果研究组血肌酐恢复正常时间、血尿消失时间、尿蛋白减少50.0%时间少于对照组,差异有统计学意义(P<0.05);两组治疗有效率对比,研究组(92.31%)高于对照组(53.85%),差异有统计学意义(P<0.05);治疗前两组Th1、Th2、Th1/Th2水平和IFN-γ、IL-4、IFN-γ/IL-4水平对比差异无统计学意义(P>0.05),经治疗,研究组优于对照组,差异有统计学意义(P<0.05)。结论连续血液净化辅助甲基强的松龙及环磷酰胺联合冲击治疗狼疮性肾炎合并狼疮性肺炎效果显著,可促使患者临床症状及早改善,提高治疗效果,改善细胞免疫功能。Objective To analyze the effect of continuous blood purification auxiliary methylprednisolone and cyclophosphamide effect on Th1/Th2 cytokine imbalance of patients with lupus nephritis complicated with lupus pneumonia.Methods 52 lupus nephritis patients complicated with lupus pneumonia were randomly divided into the study group （n=26） and the control group （n=26） from January 2010 to March 2016 in our hospital.The control group was treated with methylprednisolone and cyclophosphamide pulse methylprednisolone, and the study group was assisted by continuous blood purification of methylprednisolone combined with cyclophosphamide pulse therapy.The clinical symptoms before and after treatment, Th1 and Th2, Th1/Th2, IFN-γ, IL-4, and IFN-γ/IL-4 were recorded and compared.Results In the study group, the time of recovery of serum creatinine, the disappearance time of hematuria and the decrease of urinary protein were less than those in the control group （P〈0.05）.The treatment efficiency was higher in the study group （92.31%） than in the control group （53.85%） （P〈0.05）.There was no significant difference in Th1, Th2, Th1/Th2, IL-4, IFN-γ or IFN-γ/IL-4 before treatment between the two groups （P〉0.05）.After treatment, they were better in the study group than in the control group （P〈0.05）.Conclusion Continuous blood purification and auxiliary methylprednisolone cyclophosphamide methylprednisolone can promote the clinical symptoms, and improve the therapeutic effect and cellular immune function in treatment of patients with lupus nephritis complicated with lupus pneumonia.
