摘 要:目的探讨对轻中度稳定期慢阻肺患者采取不同强度有氧训练对运动心肺功能的影响。方法2014年8月-2015年8月在我院门诊及住院治疗的90例轻中度稳定期慢阻肺患者进行观察,根据患者能承受的最大功率将患者平均分为观察组、实验组和对照组,每组各30例。观察组采用高强度有氧训练,训练5min后休息5min在进行循环训练,每次40min,3次/周。实验组采用低强度有氧训练,每次20min,3次/周。对照组常规呼吸功能训练,胸部加压放松呼吸训练,每天两次,每次10min。在训练前后对所有患者均采用CPET试验和6分钟行走试验,评定患者的心肺功能和运动耐力情况。训练时间均为8周,观察训练前后三组患者的通气功能和运动耐力改善情况。结果经过训练后,所有患者的峰值功率、峰值通气量等肺通气功能和运动耐力结果都有了不同程度的改善,但是观察组患者的肺通气功能和运动耐力结果改善情况明显优于实验组和对照组,各组间差异有统计学意义(P<0.05)。结论轻中度稳定期慢阻肺患者采取高强度有氧训练的肺通气功能、运动耐力改善情况明显优于低强度有氧训练、常规呼吸训练,可以提高患者的肺功能,有利于患者的预后。Objective To investigate the effect of different intensity of aerobic training on pulmonary ventilation function and exercise tolerance in patients with mild to moderate stable stage of COPD.Methods From August 2014 to August 2015, 90 cases of outpatients and inpatients with mild to moderate stable COPD were observed.According to the maximum power of tolerance, they were divided into the observation group, the experimental group and the control group, 30 cases in every group.The observation group was given high intensity aerobic training, 5-minute training after 5-minute rest for a cycle, each 40min, 3 times/week.The experimental group was given low intensity aerobic training, each 20min, 3 times/week.The control group was given normal respiratory function training, chest compression to relax breathing training, twice a day, 10 minute for each time.Before and after training, all patients were evaluated with CPET test and 6 minutes walk test, respectively.Training time was 8 weeks, and their ventilation function and exercise tolerance were observed.Results After training, their peak power, peak ventilation pulmonary ventilation function and exercise tolerance results all improved, but the improvement of pulmonary ventilation function and exercise tolerance was significantly better in the observation group than in the experimental group and the control group (P〈0.05).Conclusion The high strength aerobic training of pulmonary ventilation function and exercise tolerance has better curative effect than low intensity aerobic training and regular breathing training does for patients with light, moderate and stable COPD, which can improve their pulmonary function and prognosis.
