摘  要:目的:评价不同医生利用锥形束CT(cone-beam computed tomography,CBCT)测量牙槽骨高度的可靠性。方法:选取89名下颌第一磨牙缺失的患者的种植术前CBCT,由两位医生分别利用专用NNT4.6软件,独立盲法测量下颌第二磨牙和下颌第二前磨牙之间的牙槽嵴顶中点至下颌神经管上壁的最短距离,2周后在相同条件下再次盲法测量。以一位放射学专家测量的数值为标准,比较同一检查者两次、不同检查者之间测量结果的可靠性。结果:两位医生2次测量结果的优良率分别为91%、90%和89%、90%;同一医生2次测量结果的Kappa值分别为0.841和0.840,一致性较好;两位医生之间2次测量结果的Kappa值分别为0.334和0.362,一致性较差。结论:CBCT对牙槽骨线距测量的准确性与可靠性较好,但不同观察者利用CBCT测量牙槽骨的数值可能存在较大的差异。Objective:To evaluate of the reliability of cone-beam computed tomography(CBCT) for measuring alveolar bone height by different doctors. Methods:The CBCT images of 89 patients with missed mandibular first molars were eval- uated by two independent trained examiners two times with NNT software 4. 6. Measurements were taken from the midpoint of alveolar bone crest between mandibular second molar and second premolar to the most superior part of the mandibular canal. The measurement obtained by a radiologist was taken as a gold standard ,and the resuhs of the two doctors were analysed by Kappa value. Results:The good rates of two times of each examiner were 91% ,90% and 89% ,90%. The Kappa values be- tween two times of each examiner were 0. 841 and 0. 84. The Kappa values for the agreement between the two examiners were 0. 334 and 0. 362. Conclusion:CBCT is acceptable for alveolar bone linear measurements for its good accuracy and reliabili- ty ,while it has a tendency difference when measure alveolar bone height by different doctors.
