摘  要:目的探讨右室心内膜心尖部起搏和右室间隔部起搏对儿童患者心功能的影响。方法选择2006年1月—2015年1月39例植入心脏人工永久起搏器的儿童患者,根据心室电极植入部位分为心尖部起搏(RVA组)(18例)和右室间隔部(RVS)组(21例)。随访18个月。观察两组患者术中、术后18月电极参数(感知、阈值、阻抗)的变化;比较两组患儿术前,术后18月心脏彩超,脑钠肽(BNP)数据,分析不同的起搏部位对心功能的影响。结果分析两组患者术中、术后18月电极参数,两组间数据无差异性(P>0.05)。两组患者心脏左室射血分数(LVEF),心输出量(CO),比较均无统计学差异(P>0.05)。BNP比较存在统计学意义(P<0.05)。结论儿童患者心尖部和室间隔起搏各项参数均较好。随访中未出现心脏功能降低。Objective To evaluate the influence of long - term cardiac function of right ventricular apex (RVA) pacing and right ventricular septal (RVS) pacing in children. Methods We retrospectively analyzed the clinical data of 39 children with double -chamber pacemaker implantation from March 2006 to April 2015 in Henan Provincial People' s Hospital. According to the implantation sites of ventricular electrode, the children were divided into RVA group ( n = 18 ) and RVS group ( n = 21 ). After 18 months of follow - up, compared the change of parameters ( sensory, threshold, impedance) of the two groups immediately after operation and 20 months later. Compare two groups of patients with pre- operative heart colour to exceed 20 months, the BNP data, to analyze the effects of different position on the heart func- tion. Results Analysis of two groups patients with intraoperative and postoperative 18 months electrode parameters, da- ta have no diffence (P 〉0.05). Two groups of patients with left ventricular ejection fraction (LVEF), cardiac output (CO), no statistical differences by comparing the data (P 〉 0. 05 ). The BNP figures are statistically significant (P 〈 0. 05). Conclusion Both groups of children are beter at different position parameters of the pacemaker.
