摘　　要：目的 探讨鼻唇沟皮瓣转移联合自体游离植皮修复鼻翼恶性肿瘤切除后继发鼻翼洞穿性缺损的疗效.方法 2012年1月至2015年1月共治疗鼻翼部恶性肿瘤病例9例,肿瘤完整切除后,形成鼻翼洞穿性缺损,面积1.5 cm＆#215;1.3 cm至2.5 cm＆#215;2.5 cm,均一期实施鼻唇沟皮瓣翻转为衬里联合自体游离皮片移植修复缺损,术后随访6～18个月.结果 9例患者术后翻转皮瓣及游离皮片完全成活,伤口均一期愈合,鼻翼外观与对侧基本对称,鼻腔内部略有臃肿,无明显通气功能障碍.3例行二期瘢痕修复.结论 鼻唇沟皮瓣局部翻转联合自体游离植皮是修复较大面积鼻翼洞穿性缺损可靠、有效的治疗术式.结合二期外观整形,可获得满意的手术效果.Objective To explore the feasibility of nasolabial sulcus flap transfer with autologous free skin graft to repair the alar defects after malignant tumor resection.Methods From January 2012 to January 2015,9 patients with malignant tumor were treated in the hospital.After complete tumor removal,the defect area being reconstructed was 1.5 cm ＆#215; 1.3 cm to 2.5 cm ＆#215; 2.5 cm.The defects of 9 patients were all restored with nasolabial sulcus flap combined with autologous free skin graft.Results The 9 patients were followed up for 6-18 months postoperatively.The nasolabial sulcus flap and autologous free skin graft were survived completely in all cases.Symmetrical alae were noted with slight edema within nasal cavity but without difficult ventilation.Scar was repaired in phase-two surgery.Conclusions Nasolabial sulcus flap combined with autologous free skin graft is an optional way in alar defects restoration.Further with secondary morphologic plasty,satisfactory surgical outcome can be achieved.
