主管切除高位置管支管对口引流术治疗高位复杂性肛瘘的疗效  

Efficacy of Main Tract Resection Setting Tube in the High Position Branch Tract Counter-opening Drainage for Treating High Complex Fistula

摘　　要：为探讨主管切除、高位置管、支管对口引流术治疗高位复杂性肛瘘的临床疗效,选取2013年5月至2014年4月于我院拟行手术治疗的60例高位复杂性肛瘘患者进行研究,随机分为观察组和对照组,各30例.观察组行主管切除、高位置管、支管对口引流术,对照组行切开挂线术,对比2组患者治愈率、创面愈合时间、术后肛门功能评分及复发情况.结果显示,2组患者治愈率比较差异无统计学意义,P〉0.05;观察组创面愈合时间明显短于对照组,P〈0.05;观察组术后肛门功能明显优于对照组,P〈0.05;术后1年复发率明显低于对照组,P〈0.05.结果表明,主管切除、高位置管、支管对口引流术治疗高位复杂性肛瘘治愈率高,患者术后恢复快,对肛门功能影响小,复发率低,临床应用价值高.In order to explore the clinical efficacy of main tract resection,setting tube in the high position and branch tract counter-opening drainage in the treatment of high complex anal fistula, 60 patients with high complex anal fistula treated in authors＇＇ hospital from May 2013 to April 2014 were selected and randomly divided into observation group and control group, 30 cases for each.The observation group underwent main tract resection,setting tube in the high position and branch tract counter-opening drainage while the control group accept incision thread-drawing procedure.Curative rate, wound-surface healing time, postoperative anal function score and recurrence of the two groups were compared.As results, in the curative rate of the two groups, there was no statistical difference （P〉0.05）;the wound-surface healing time of the observation group was significantly shorter than that of the control group （P〈0.05）;in the postoperative anal function, the observation group was significantly better than the control group （P〈0.05）;The recurrence rate within one year after operation of the observation group was significantly lower than that of the control group （P〈0.05）.In conclusion, main tract resection,setting tube in the high position and branch tract counter-opening drainage in the treatment of high complex anal fistula has high curative rate and faster recovery.It has less influence on anal function and low recurrence rate, having high clinical application value.
