摘  要:目的观察雌激素对2型糖尿病缺血性心肌病去卵巢大鼠代谢的影响,探讨其对糖尿病缺血性心肌病的保护作用。方法建立2型糖尿病缺血性心肌病去卵巢大鼠模型,不同条件饲养3个月后,测定各组大鼠血清性激素结合球蛋白(SHBG)、雌激素、血糖、血脂及胰岛素水平,观察雌激素替代治疗(ERT)对各指标的影响。结果与对照组比较,糖尿病组和冠心病组血清SHBG和雌二醇(E2)水平明显降低,促卵泡激素(FSH)和促黄体激素(LH)水平均明显升高(P﹤0.05),而糖尿病组与冠心病组的血清SHBG和雌激素水平无显著性差异(P>0.05);与糖尿病组和冠心病比较,治疗组血清SHBG和E2水平明显上升,而FSH和LH水平均明显降低(P﹤0.05)。与对照组比较,糖尿病组和冠心病组血LDL、TG、空腹血糖、胰岛素水平明显升高,HDL明显降低(P﹤0.05),而糖尿病组与冠心病组的这些指标均无显著性差异(P>0.05);与糖尿病组和冠心病组比较,治疗组LDL和FINS下降(P﹤0.05),HDL明显上升(P﹤0.05),而TG无显著改变(P>0.05)。结论 ERT可以降低糖尿病缺血性心肌病模型大鼠血糖,改善血脂比例,增加胰岛素敏感性。Objective To observe the impacts of estrogen on metabolism in ovariectomized rats with type 2 diabetes mellitus (T2DM) ischemic cardiopathy and explore its role in the protection of diabetic ischemic cardiomyopathy. Methods Models of ovariectomized rats with T2DM ischemic cardiopathy were established, and the rats were fed for three months under different conditions; and then, the serum sex hormone binding globulin (SHBG), estrogen, blood sugar, blood lipid and insulin levels in rats in each group were detected to observe the impacts of estrogen replacement therapy (ERT) on various indexes. Results Compared with the control group, the SHBG and estradiol(E2) levels in T2DM group and coronary heart disease (CHD) group decreased greatly, while follicle stimulating hormone(FSH) and luteinizing hormone(LH) levels increased significantly (P 〈 0.05); the SHBG and estrogen levels between the T2DM group and the CHD group had no significant difference (P 〉 0.05); compared with T2DM group and CHD group, the SHBG and E2 levels in the treatment group increased greatly, while FSH and LH levels decreased significantly (P 〈 0.05). Compared with the control group, the blood low-density lipoprotein (LDL), TG, fasting blood glucose and insulin levels in T2DM group and CHD group increased significantly, while HDL decreased greatly(P 〈 0.05). These indexes between the T2DM group and the CHD group showed no significant difference (P 〉 0.05); compared with T2DM group and CHD group, the LDL and FINS in the treatment group decreased (P 〈 0.05), HDL increased greatly (P 〈 0.05), and TG had no significant change (P 〉 0.05). Conclusion ERT can lower the glucose in rats with diabetic ischemic cardiopathy, improve blood lipid ratio and increase insulin sensitivity.
