参附注射液联合血必净注射液治疗脓毒症休克患者炎症指标和血流动力学临床分析

摘　　要：目的探讨参附注射液联合血必净注射液治疗脓毒症休克的临床效果及其对患者炎症指标——血乳酸(Lac)、血清降钙素原(PCT)、C反应蛋白(CRP)及血流动力学的影响。方法选择该院重症医学科2013年1月至2016年6月收治的脓毒症休克患者168例,采用随机数字表法分为研究组和对照组,每组84例。对照组仅采用常规治疗方法 ,研究组在常规治疗方法基础上加用参附注射液联合血必净注射液治疗,比较两组患者临床疗效,同时检测治疗前及治疗24、72 h后Lac、PCT、CRP水平及血流动力学指标。结果研究组总有效率明显高于对照组,治疗后Lac、PCT、CRP水平较治疗前明显降低,且降低程度明显大于对照组,治疗后血流动力学指标明显优于治疗前,且改善程度明显大于对照组,差异均有统计学意义(P<0.05)。结论参附注射液联合血必净注射液治疗脓毒症休克疗效显著,对Lac、PCT、CRP水平及血流动力学影响较大,值得临床推广应用。Objective To investigate the clinical effect of Shenfu Injection combined with Xuebijing Injection in treating septic shock and its influence on blood inflammatory indexes lactic acid （Lac）, serum procalcitonin （PCT）, CRP levels and hemodynamics. Methods One hundred and sixty-eight patients with septic shock in the intensive care medicine department of our hospital from January 2013 to June 2016 were selected and divided into the study group and control group according to the random number table method, 84 cases in each group. The control group only adopted the routine therapy. On this basis the study group was added with the Shenfu Injection combined with Xuebijing Injection. The clinical effects were compared between the two groups. The levels of blood Lac,serum PCT and CRP levels and hemodynamic indexes were measured before treatment and at 24,72 h after treatment. Results The total effective rate of the study group was significantly higher than that of the control group. The levels of Lac, PCT and CRP after treatment were significantly decreased compared with before treatment, moreover the decrease degree in the study group was obviously more than that in the control group. The hemodynamic indexes after treatment were significantly superior to those before treatment, moreover the improvement degree in the study group was significantly more than that in the control group, the difference was statistically significant （P〈0.05）. Conclusion Shenfu Injection combined with Xuebijing Injection has significant effect in the treatment of septic shock, has a great influence on blood Lac, PCT and CRP levels and hemodynamics, and deserves to be popularized and applied in clinic.
