摘 要:目的探讨CT动态灌注成像技术测定肝脏血流量与肝硬化程度的关系。方法选取50例肝硬化患者作为研究组,再选择同期入院体检的健康者26例作为对照组。两组研究对象均同时进行含有肝脏、脾脏、主动脉、门静脉的层面的CT动态灌注成像检验。结果与对照组比较,研究组的肝硬化患者的HAP、PVP和THBF均明显降低,MTT有所延长,组间比较,差异具有统计学意义(P<0.05),但两组HPI和DV比较,差异不具有统计学意义(P>0.05);肝硬化程度不同HAP、PVP、THBF、HPI和MTT比较,差异具有统计学意义(P<0.05)。结论使用螺旋CT灌注成像测定肝脏血流量具有无创、安全、重复性好等特点,可在临床范围内广泛使用。Objective To investigate the relationship between hepatic blood flow and the degree of liver cirrhosis by CT dynamic perfusion imaging. Methods 50 patients with liver cirrhosis were selected as the study group, and 26 health people were selected at the same period of physical examination as the control group. Both groups were selected to include the liver, spleen, aorta, portal vein at the level of CT dynamic perfusion. Results Compared with the control group, the levels of HAP, PVP and THBF were significantly decreased in the study group, MTT was prolonged. The difference between the two groups was statistically significant(P〈0.05), but the HPI and DV in two groups were not significantly different(P 〉0.05). There were significant differences in HAP, PVP, THBF, HPI and MTT in different degree of liver cirrhosis, the difference was statistically significant(P〈0.05). Conclusion The use of spiral CT perfusion imaging in the detection of liver blood flow is non-invasive, safe, good repeatability and other characteristics. It is recommended to be widely used in the clinical context.
