摘 要:目的探讨高血铅对男性精液质量及性激素水平的影响。方法选择156例高血铅男性患者为高铅组,对照组为180例已生育健康正常男性。采用SQA—V型精子质量分析系统对队上两组实验对象精液标本的各项主要参数进行对比分析,同时采用化学发光法进行血清泌乳索(PRL)、卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)和雌二醇(E2)检测。结果高铅组男性的精子密度、活率、前向运动率及正常精子形态率分别为(54.16±18.89)×10^6/ml、(42.47±15.72)%、(26.50±10.08)%和(9.67±6.12)%,均明显低于对照组的(68.33±27.81)×10^6/ml、(57.90±19.87)%、(35.03±8.82)%和(13.38±8.25)%;高铅组男性血清FSH和LH分别为(11.57±5.19)mlU/ml、(9.76±4.05)mlU/ml,明显高于对照组的(7.50±3.26)mlU/ml、(5.41±2.53)mIU/ml;高铅组男性T水平为(2.31±1.16)ng/ml,明显低于对照组的(6.97±2.28)ng/ml;两组比较差异均有统计学意义(均P〈0.05)。结论高血铅会对男性精液质量及性激素水平产生不良影响,造成男性生育能力下降。Objective To explore the effect of high blood lead level on semen quality and sex hormones levels in males. Methods 156 male patients with high blood lead level were selected as the high blood lead group, and 180 healthy fertile men as the control group. Compared the main parameters of semen samples from the two groups using SQA-V Sperm Quality Analyzer, and the chemiluminescence was used to detect the serum prolactin (PRL), follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), and estradiol (E2). Results The sperm density, motility, forward motility rate, and morphology rate of normal sperm in the high blood lead group were (54.16± 18.89)× 10^6/ml, (42.47± 15.72)%, (26.50± 10.08)%, and (9.67±6.12)% respectively, which were all significantly lower than those in the control group [(68.33±27.81)×10^6/ml, (57.90±19.87)%, (35.03±8.82)%, and (13.38±8.25)%]; serum FSH and LH levels in the high blood lead group were (11.57±5.19) mIU/ml and (9.76±4.05) mIU/ml, respectively, which were all significantly higher than those in the control group [(7.50±3.26) mIU/ml and (5.41±2.53) mIU/ml]; T level in the high blood lead group was (2.31±1.16) ng/ml, which was significantly lower than that in the control group [(6.97±2.28) ng/ml]; there were statistically significant differences between the two groups (all P〈0.05). Conclusion High blood lead level will have a negative effect on male semen quality and serum sex hormones levels, resulting in male fertility decline.
