Sextant内固定技术在野战手术救治方舱内治疗胸腰椎骨折的应用研究

摘　　要：目的探讨经皮Sextant内固定技术在野战手术救治方舱治疗无神经损伤的胸腰椎骨折的近期临床疗效。方法回顾性分析32例沈阳军区总医院骨科参与救治的无神经损伤胸腰椎骨折病例,其中男18例、女14例,平均年龄(25.54±2.86)岁。本组病例均于沈阳军区总医院研制的野战手术救治方舱内接受经皮Sextant内固定技术。观察患者手术时间、术中失血量、术后下床时间及切口愈合情况,同时对比观察患者术前、术后3 d的腰痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分、伤椎的椎体前缘高度、局部后凸Cobb角。结果平均手术时间(96.55±14.15)min、术中平均失血量(45.25±3.55)m L、术后患者平均下床时间(1.20±0.61)d,术后3 d腰痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分、伤椎椎体前缘高度及局部后凸Cobb角较术前均得到明显改善,且差异均有统计学意义(P<0.05)。结论在野战手术救治方舱中行经皮Sextant系统内固定术能对无神经损伤的胸腰椎骨折早期及时、安全、有效的救治,可应用于战争或灾难现场紧急救治。Objective To discuss the short-term effect of Sextant system fixation tor thoracolumbar fractures without neurological deficit in the field comprehensive operation rescue shelter. Methods There were 32 patients with thoracolumbar fractures without neumlogieal defi- cit which rescued by our hospital. Those patients included 18 males and 14 females, with average age of （25.54 ＋2.86） years old. All patients were undergoing internal fixation with Sextant system in the field comprehensive operation rescue shelter. Clinical and surgical evalua- tion including surgery time,intraoperative blood loss,postoperative ambulation time and wound healing time were observed. Functional out- comes of pre-operation and postoperation 3-days were evaluated by visual analog scale（VAS） and Osweswy disability index（ODI）. Cobb an- gle and front height of fi＇acture vertebral body were compared between pre-operation and postoperation 3-days. Results The operation time was （96.55 ＋ 14.15 ） minutes, intraoperative blood loss was （45.25 ＋ 3.55 ） mL, postoperative ambulation time was （ 1. 20 ＋ 0.61 ） days. There were statistical diftrences in terms of Cobb angle, fi＇ont height of fracture vertebral body, VAS and ODI between pre-operation aml post- operation 3 days. Conclusion The Sextant system fixation in the field comprehensive operation rescue shelter is a timely,safe and effective method for thoracolmnbar fractures without neurological deficit,which is deserved to utilize at the scene when war or disaster urgency treatulent.
