摘  要:目的探讨溃疡性结肠炎(ulcerative colitis,UC)病人体内维生素C(vitamin C,VC)和维生素D(vitamin D,VD)水平对炎症程度的影响。方法收集2016年1月至2016年12月在山西省大同市同煤集团总医院确诊的40例缓解期UC患者,采用高效液相色谱(high performance liquid chromatography,HPLC)紫外检测法检测血清中VC水平,根据VC水平将其分为高VC组(2~4mg/L)和低VC组(<2mg/L)。采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)和比色法检测血清中25-(OH)D_3、1,25-(OH)_2D_3、TNF-α、IL-6、IL-1β和髓过氧化物酶(myeloperoxidase,MPO)、谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)的水平,比较两组间的差异。结果高VC组25-(OH)D_3、1,25-(OH)_2D_3水平高于低VC组,高VC组25-(OH)D_3与低VC组有统计学差异(P<0.05)。高VC组血清中TNF-α、IL-6、IL-1β、MPO水平低于低VC组,差异均有统计学意义(P<0.05)。高VC组血清GSH-Px高于低VC组,尚无统计学差异。结论 UC患者血清VC水平影响25-(OH)D_3和1,25-(OH)_2D_3的水平,表明VC可能参与VD的活化,在适宜范围内VC与25-(OH)D_3、1,25-(OH)_2D_3呈正相关,1,25-(OH)_2D_3通过免疫机制参与UC的发病和发展,与UC的严重程度呈负相关。Objective To investigate effects of vitamin C and vitamin D levels on the severity of inflammation in patients with ulcerative colitis. Methods 40 cases of UC patients who were in remission were collected in With the Coal Group General Hospital,Datong,Shanxi Province from January 2016 to December 2016. The level of VC in the serum was detected by high - performance liquid chromatography with UV detection. They were divided into high VC group (2 -4mg/L) and low VC group (〈2mg/L) according to the level of VC. Levels of 25 - (OH) D3, 1,25 - (OH)2 D3, TNF -α, IL - 6, IL - 1β and myeloperoxidase ( MPO ), glutathione peroxidase ( GSH - Px ) were detected by enzyme - linked immunosorbent assay ( ELISA ) and colorimetry. Then we compared the difference between the two groups. Results Levels of 25 - (OH) D3,1,25 - (OH) 2 D3 in the high VC group were higher than those in the low VC group, and the difference of 25 - (OH)D3 was statistically significant ( P 〈 0.05 ). Levels of TNF - α, IL - 6, IL - 1β and MPO in the high VC group were lower than those in the low VC group, and the difference was statistically significant ( P 〈 0.05 ). The GSH - Px in the serum of the high VC group was higher than that of the low VC group, but there was no statistical difference. Conclusion The level of VC in patients with UC affected the levels of 25 - (OH) D3 and 1,25 - (OH)2 D3 in the serum. It suggested that the level of VC may participate in the activation of VD. The level of VC was positively correlated with 25 - (OH) 93 and 1,25 - (OH)2 D3 in the appropriate range. 1,25 - (OH)2D3 was involved in the pathogenesis and development of UC through the immune mechanism and negatively correlated with the severity of UC.
