摘 要:目的:探讨等离子低温射频消融术联合鼻内镜对慢性鼻—鼻窦炎(CRS)患者鼻通气及鼻腔黏液纤毛清除能力的影响。方法:选择2015年8月至2016年8月在安徽省六安市人民医院接受治疗的96例CRS患者,按随机数字表法将其分为研究组(等离子低温射频消融术联合鼻内镜手术)和对照组(鼻内镜鼻窦手术),每组48例,比较两组患者的临床疗效、Lund-Kennedy评分、SNOT-20评分、嗅觉功能、鼻腔黏液纤毛清除能力和鼻通气功能情况。结果:术后3个月,研究组临床总有效率和嗅觉功能正常率分别为93.75%和64.58%,高于对照组的79.17%和39.58%(均P<0.05);研究组Lund-Kennedy评分、SNOT-20评分、糖精清除时间、鼻阻力、鼻腔最小横截面积到前鼻孔距离均低于对照组(均P<0.05);而研究组黏液纤毛清除速率、黏液纤毛清除率、鼻腔最小横截面和鼻腔容积均明显大于对照组(均P<0.05)。结论:等离子低温射频消融术联合鼻内镜可明显提升CRS患者临床治疗总有效率,显著改善患者术后临床症状,有利于患者嗅觉功能、鼻腔通气及鼻黏液纤毛清除能力的恢复。Objective: To explore the effect of low temperature plasma radiofrequency ablation combined with endoscopic sinus surgery on the nose ventilation and nasal mucous ciliary clearance capacity of chronic rhinosinusitis (CRS). Methods: 96 CRS patients treated in our hospital from August 2015 to August 2016 were recruited, and divided into a control group (endoscopic sinus surgery) and a research group (low temperature plasma radiofrequency ablation combined with endoscopic sinus surgery) randomly with 48 case in each group. The clinical efficacy, Lund-Kennedy score, SNOT 20 score, olfactory function, nasal mucous ciliary clearance capacity and nose ventilation were compared. Results: Three months after surgery, the to tal effective rate and normal ratio of olfactory function in research group were 93.75% and 64.58% respec tively, which were significantly higher than those in the control group (79.17% and 39. 58% ) (P〈0.05). The Lund-Kennedy score, SNOT 20 score, saccharin clearance time, nasal resistance and the distance of the minimal cross-sectional area to the nostril in the research group were significantly lower than those in the control group (P〈0.05). The mucociliary clearance, mucociliary transport rate, nasal minimal cross sectional area and nasal cavity volume in the research group were significantly higher than those in the con troi group (P〈0.05). Conclusion: Low temperature plasma radiofrequency ablation combined with endoscopic sinus surgery could significantly elevate the clinical total effective rate of CRS, ameliorate postoperative clinical symptom, and improve the olfactory function, nose ventilation and nasal mucous ciliary clear ance capacity.
