分期行腹腔镜手术治疗阑尾周围脓肿的研究

摘　　要：目的:探讨分期行腹腔镜手术治疗阑尾周围脓肿的效果。方法:将2014年6月-2016年6月本院收治的120例阑尾周围脓肿的患者采用随机数字表法分为Ⅰ期组和Ⅱ期组,每组60例。Ⅰ期组实行急诊手术治疗,Ⅱ期组先予抗感染等保守治疗,待3个月后返院实行腹腔镜阑尾切除术,比较两组术后首次排气时间、总住院天数、手术时间、术后并发症发生率及住院治疗总费用。结果:Ⅰ期组患者手术时间长于Ⅱ期组,总住院天数及住院费用均少于Ⅱ期组,比较差异均有统计学意义(P<0.05);两组术后首次排气时间及术后并发症比较,差异均无统计学意义(P>0.05)。结论:Ⅰ期行腹腔镜阑尾切除术的患者较Ⅱ期费用更低廉,住院时间更短,同时并不增加手术治疗的风险性,可以取得显著的治疗效益,值得临床推广应用。Objective： To observe the effect Of staging laparoscopic treatment periappendicural absecess. Method： From June 2014 to June 2016, 120 patients with periappendicural absecess treated in our hospital were selected, and divided into stage Ⅰgroup and stage Ⅲ group according to the random number table method, 60 cases in each group.stage Ⅰ group was adopted emergency, stage Ⅲ group was adopted conservative treatment like anti-infective, then received laparoscopic appendectomy after 3 months.The postoperative first exhaust time, hospitalization days and operation time, postoperative complications occurrence condition and total hospitalization cost of two groups were compared.Result： The operation time of stage Ⅰ group was longer than that of stage Ⅱ group, the total hospitalization days and the hospitalization cost of stage Ⅰ group both were less than those of stage Ⅱ group, the differences were statistically significant （P〈0.05） .The postoperative first exhaust time and postoperative complications occurrence rate of two groups were no differences （P〉0.05） .Conclusion： Patients adopted stage Ⅰ laparoscopic appendectomy compared with who adopted stage Ⅱ laparoscopic appendectomy, has lower cost and shorter hospitalization time, which not increase the risk of surgery, has obvious treatment effect, worthy of clinical popularization and application.
