紧急宫颈环扎术在晚期流产和早产临产中的临床效果观察

摘　　要：目的评估紧急宫颈环扎术在治疗因宫颈机能不全致晚期流产和早产临产中的效果。方法选择2014年1月至2015年12月我院产科32周前因宫颈机能不全致晚期流产和早产临产患者共76例,其中环扎组45例采取紧急宫颈环扎术联合宫缩抑制剂,对照组21例仅予宫缩抑制剂,比较两组患者的治疗情况。结果环扎组保胎天数多于对照组,两组不同孕周分娩率比较差异有统计学意义(P<0.05);环扎组盐酸利托君抑制宫缩效果优于传统药物硫酸镁,差异有统计学意义(P<0.05)。结论对宫颈机能不全致晚期流产或早产临产患者采用紧急宫颈环扎术联合宫缩抑制剂治疗效果更好,增加34周以上的分娩率,提高新生儿存活率。Objective To evaluate the effect of emergency cervical cerclage in the treatment of late abortion and preterm labor due to cervical incompetence. Methods We divided 76 cases, who had late abortion and preterm labor in 32nd gestational weeks in- duced by cervical incompetence in our hospital between January 2014 and December 2015,into treatment group （n = 45 ） and control group （n = 21 ）. The treatment group accepted the emergency cervical cerclage and tocolytics, while the control group accepted tocolyt- ics only. The clinical effect of the treatments was compared between the two groups. Results The days of prolonged gestation in the treatment group were longer than that in the control group. The delivery rate in various gestation weeks was significantly different be- tween the two groups （ P 〈 0. 05 ）, and the treatment group was better than the control group （ P 〈 0.05 ）. Conclusion The emergency cervical eerclage and tocolytics have a better clinical effect to patients with late abortion and preterm labor induced by cervical incompe- tence. It can increase the delivery rate after 34th gestation week and improve neonatal livability.
