摘 要:目的调查肛周脓肿患者感染的病原菌分布特点以及主要致病菌的耐药情况,为经验性选择抗菌药物治疗肛周脓肿患者提供理论依据。方法回顾性抽查我院2014年1月~2015年12月肛周脓肿标本355例,对细菌培养、鉴定及药敏试验结果进行统计分析。结果从肛周脓肿患者脓液中共检出细菌274株,检出菌中大肠埃希菌163株,占59.49%,其对亚胺培南的敏感率为100%,对阿米卡星、哌拉西林/他唑巴坦、头孢替坦、头孢吡肟的敏感率高达90%以上,对其余抗菌药物均有一定程度耐药性;经验性选择头孢唑肟治疗肛周脓肿的有效(治愈+好转)率为96.19%。结论大肠埃希菌是我院肛周脓肿患者的主要致病菌,科室应根据药敏结果指导医生合理地经验性使用抗菌药物。Objective To survey the distribution characteristics of pathogenic bacteria in crissum abscess patients as well as the resistance of the main pathogenic bacteria,and to provide a foundation for empiric antimicrobial treatment in crissum abscess patients.Methods A total of 355 patients with crissum abscess who enrolled in hospital from Jan.2014 to Dec.2015 were included.Pus specimens were collected for routinely microbiological culture identification and drug susceptibility test,and the results were analyzed.Results A total of 274 strains bacteria were detected from the pus of crissum abscess.163 strains of E.coli were checked out,accounting for 59.49%.The sensitive rate for imipenem was 100%,and the sensitive rate for piperacillin-tazobactam amikacin,cefotetan,and cefepime were all above 90%.The effective rate(cured+improved)of cefazolin oxime treatment was 96.19%.Conclusion E.coli was the main pathogenic bacteria in crissum abscess patients.The departments should guide doctors applying antimicrobial agents reasonably according to the results of drug susceptibility test.
